Over-exertion (Not Elsewhere Classified)

I was cleaning the 2nd deck of a 3 deck screen with a bar. The bar got lodged between 2 wires of the screen, I hurt my left shoulder dislodging it.

Struck against stationary object

Pulling the cord to start the pony engine for the crusher and the cord broke causing ee to slam elbow into the step rail causing a cut. EE went to health reach and received stitches. Employee elected to take a sick day the next day.

Caught in, under or between a moving and a stationary object

material was stuck to apron screen deck that drops into the cone. Employee was cleaning the material off with bar, swung bar too far in to get to the back corner and got top of tampering bar too close to overhead conveyor belt. Bar was grabbed by belt and pulled in, thumb was pinched between bar and cross member and resulting in the tip of the thumb being pulled off.
