Employee was assisting with loading trucks at the lime plant. He stepped into a mud puddle, causing his feet to get wet. This resulted in lime burns to both of his feet. Employee did not have on the required rubber boots to work in that area. Employee did not report injury until 2/09/06.

After completing a weld, employee lifted welding helmet and a piece of slag slid off of helmet, behind safety glasses hitting employee's eye. Employee did not report incident until December 30, 2004 when he stated that it felt as though there were a piece of hair in his eye.
Another employee improperly lifted hopper onto ends of forklift causing it to fall off and tip over. The hopper hit injured employee's knee causing laceration.
