While attempting to restore trunnion water via a systematic check of the hoses, a hose was removed causing scalding water to burn face.

EMPLOYEE WAS TICHTENING NUT ON ROCK RETURN ELEVATOR WHEN WRENCH SLIPPED AND CUT MIDDLE FINER ON RIGHT HAND.

EMPLOYEE WAS WALKING OVER TO LOOK AT THE COAL TUNNEL AND TRIPPED OVER A HOSE AND TWISTED ANKLE

EMPLOYEE USED BOARD TO DISLODGE AUGER JAM. BOARD TWISTED, HIT HAND AND PULLED SKIN OFF HAND

EMPLOYEE HAMMERING ON PLATFORM, LOST BALANCE, SLIPPED AND FELL TO FLOOR - APPROX 2 FEET.

CLOSING LIDS ON TRUCK WHEN WIND BLEW PARTICLES IN LEFT EYE

EMPLOYEE WAS SHOVELING WET FINES AND WENT TO THROW IT AND FELT A PAIN IN LOWER GROIN

HE WAS GOING DOWN STEPS AND SLIPPED AND FELL; TWISTED KNEE AND IT BEGAN SWELLING.

EMPLOYEE WAS SHOVELING AND HIT ROCKS WITH SHOVEL, JARRED BODY AND PULLED MUSCLE IN BACK. THE SHOVELING AGGRAVATED THE MIDDLE OF BACK, ALSO HURT WHILE BENDING TO PICK UP ANYTHING.

While shoveling at "A" elevator employee fell and twisted his right knee.

While bricking the kiln, bricks fell from overhead breaking employee's right pinky finger.

While on the cooler floor employee got a lime burn on his left leg

While working at the kiln, lime dust blew into employee's right eye.

While cleaning out the bottom of A elevator, employee's feet got wet causing them to get lime burns. Employee did not report injury until 03/13/2006.

Employee suffered a strain to the rhomboid and trapezius muscles of his upper back while shoveling. He further aggravated it by bagging and stacking 50# bags. Employee did not report injury until 2/22/2006.

Employee was assisting with loading trucks at the lime plant. He stepped into a mud puddle, causing his feet to get wet. This resulted in lime burns to both of his feet. Employee did not have on the required rubber boots to work in that area. Employee did not report injury until 2/09/06.

While emptying hopper on cooler floor, wind picked up causing employee's left eye to get dusted with lime. Employee did not report injury until 12/02/05.

While working in the processing area, employee stepped into a mud hole causing wet lime to get into his rubber boots burning his left foot. Employee proceeded to continue working rather than address the situation. Employee did not report until 9/28/05.

Employee did not report injury until 7/25/05. Employee was shoveling trying to get a belt free. He was sweating profusely causing a lime burn on his bilateral buttock area.

Employee did not report injury until 7/21/05. Employee slipped while walking through the processing area causing water and lime to get inside his steel toe rubber boots. Employee cleaned out boot, and returned to work.

While working in the plant, employee's foot got wet causing a lime burn. Employee did not report injury until 2/11/05

While cleaning in plant, dust/debris blew into employee's eye causing irritation - Eye was irrigated and debris removed

After completing a weld, employee lifted welding helmet and a piece of slag slid off of helmet, behind safety glasses hitting employee's eye. Employee did not report incident until December 30, 2004 when he stated that it felt as though there were a piece of hair in his eye.

Step turned over causing rail to hit employee in lower back. Employee did not request medical treatment until 9/3/04.

While working, employee's foot got wet and was burned

Another employee improperly lifted hopper onto ends of forklift causing it to fall off and tip over. The hopper hit injured employee's knee causing laceration.

While working, employee began to sweat causing lime burn to stomach, legs, rt. wrist.

Employee smashed his right ring finger while pouring sand into a hopper

EMPLOYEE BAGGING ON PACKER WHEN IRRITATION FROM UNCOVERED ARMS AND BLISTER ON HAND CAUSED LIME BURNS.

EMPLOYEE BAGGING LIME ON PACKER. DUST PUFFED INTO EYE. EMPLOYEE ADMITTED HE WAS NOT WEARING SAFETY GLASSES.

Employee greasing the cooler. Lime dust got under safety glasses per employee. Employee reported this injury after he was terminated.

EMPLOYEE SHOVELING AND CLEANING UP AT KILN AREA. SHOVEL SLIPPED & FINGER SMASHED BETWEEN SHOVEL & ENGINE. FINGER DID NOT APPEAR TO BE BROKEN BUT WAS SWOLLEN. EE REFUSED TO TAKE DRUG TEST & THEN REFUSED MEDICAL TREATMENT. APPARENTLY QUITTING.

EMPLOYEE CLEANING GREASE OFF MOTOR BASE. ANGLE IRON USED TO SCRAPE WITH & IT SLIPPED & HIT EE ON HIS ARM. NO FRACTURE, BRUISED ONLY BUT EE HAD BROKEN ARM IN SAME AREA PRIOR TO WORKING HERE & WE DID NOT WANT TO TAKE ANY CHANCES. EE RELEASED TO RETURN BUT HAS NOT DONE SO.

EMPLOYEE WAS STANDING AND SHOVELING IN A CLEANUP AREA WHEN HE SLIPPED & HIT KNEE. EE DISREGARDED INSTRUCTIONS ON PROPER WAY TO SAFELY PERFORM JOB ASSIGNED.

EMPLOYEE WAS SHOVELING WHEN THUMB STARTED HURTING. NO REASON OR KNOWLEDGE OF ANY ACTION WHICH CAUSED INJURY. POSSIBILITY EXISTS THE EE HAD A PREVIOUS INJURY PRIOR TO EMPLOYMENT AT MINE THAT WAS AGGRAVATED BY SHOVELING.

BAGGING LIME - BURNT ARM.

EMPLOYEE WALKING DOWN RAMP, SLIPPED ON ICE AND FELL BENDING THUMB BACKWARDS.

EE CUT FINGER WHILE WORKING ON ASH PUMP. EE OFFERRED MEDICAL CAR BUT DECLINE.

EE SLIPPED ON STEPS GOING TO COOLER FLOOR. INDICATED TO SUPERVISOR ON SHIFT HE DID NOT NEED MEDICAL ATTENTION. CONFIRMED WITH EE THE SAME DATE. ON FRIDAY 10-15-01. EE WENT TO OWN DR. REINSTRUC TED EE ON PROCEDURE AND HE SAW CO. DR. ON 10-8-01.

CONTRACT EE WORKING IN BURNING PLANT-LIME GOT INTO BOOT, CAUSING LIME BURN ON TOE.

EMPLOYEE HAD STEPPED OFF IN LIME PUDDLE FEW DAYS BEFORE. EMPLOYEE REQUESTED TO HAVE LIME BURN CHECKED OUT AS IT HAD BEEN IRRITATED BY HIS WORKBOOT RUBBING. DR VISIT RESULTED IN PRESCRIPTION BE ING ISSUED. EMPLOYEE RETURNED TO WORK - NO LOSS TIME.

EE WAS ROLLING A V-BELT ON A SHEAVE WHEN FELLOW EE STARTED EQUIPMENT. THUMB CAUGHT IN BELT AND PINCHED IT. THE EE WAS RETRAINED IN PROPER LOCKOUT AND GUARDING PROCEDURE ALONG WITH FELLOW EMPLO YEE.

ON A RAINY DAY EMPLOYEE GOT WET LOADING A TRUCK THEN RESUMED BAGGING LIME. HE NOTICED A BURNING SENSATION BUT DIDN'T TAKE PROPER PRECAUTIONS RESULTING IN LIME BURN. EE RETRAINED IN PREVENTING LIME BURNS. NO LOSS TIME WOULD HAVE OCCURED IF PROPERLY REPORTED TO EMPLOYER.

EE RESET SWITCH AND WHEN HE REENGAGED THE POWER IT FLASHE. EE DID NOT HAVE COVER CLOSED ON BOX.

EE EXPERIENCING BACK PAIN DOES NOT KNOW EXACTLY WHEN.

EE WOKE UP WITH SEVERE BACK PAIN AS A RESULT OF TWISTING & TOSSING BRICK INTO KILN.

EE SLIPPED AND FELL ON ICE LANDING ON SHOULDER.
