Employee suffered a strain to the rhomboid and trapezius muscles of his upper back while shoveling. He further aggravated it by bagging and stacking 50# bags. Employee did not report injury until 2/22/2006.

Employee was assisting with loading trucks at the lime plant. He stepped into a mud puddle, causing his feet to get wet. This resulted in lime burns to both of his feet. Employee did not have on the required rubber boots to work in that area. Employee did not report injury until 2/09/06.

While emptying hopper on cooler floor, wind picked up causing employee's left eye to get dusted with lime. Employee did not report injury until 12/02/05.

After completing a weld, employee lifted welding helmet and a piece of slag slid off of helmet, behind safety glasses hitting employee's eye. Employee did not report incident until December 30, 2004 when he stated that it felt as though there were a piece of hair in his eye.

ON A RAINY DAY EMPLOYEE GOT WET LOADING A TRUCK THEN RESUMED BAGGING LIME. HE NOTICED A BURNING SENSATION BUT DIDN'T TAKE PROPER PRECAUTIONS RESULTING IN LIME BURN. EE RETRAINED IN PREVENTING LIME BURNS. NO LOSS TIME WOULD HAVE OCCURED IF PROPERLY REPORTED TO EMPLOYER.
