Forklift driver was backing up after putting pallet down; looked over left shoulder, looked over right shoulder, not seeing anyone, began to back up. Light and alarm were active. Heard EE yell as forklift tire ran over foot.
Cleaning around 202 conveyor, employee went to move palletizer out of way but it was too heavy. Did not feel pain until next day. Notified employer on 4/25/05.

EMPLOYEE COMPLAINED OF SORE RIGHT ARM AFTER STACKING 100# BAGS ON PALLETS. REFUSED TO GO TO DOCTOR. ON 3-19-01, HE LEFT WORK MID DAY AND RETURNED WITH DOCTOR'S NOTE RECOMMENDING NO HEAVY LIFTI NG UNTIL 4-5-01. HAS NOT RETURNED YET.
