Showing 50 of 89Rock was wedged in the conveyor belt. The on/off switch was bumped to try to free it. Did not say "go", EE said "No". When the conveyor bumped forward the rock bar was in the tail pully resulting in foreman being struck in the chest with the bar throwing them 6-7' in air and landing on the ramp.
Miner was approaching jobsite 3 hours late (10:00am) and just about to start on tasks, and they said they had back pain and needed to go to the office to go to the Dr. Never worked or did anything.
Service Technician was performing maintenance on an impact crusher, rotating the impact blow bars. While handling one of the bars EE lost grip and it slipped out of EE's hand. A sharp edge on the blow bar cut the palm of EE's hand, requiring 5 stitches to close the wound. It was a very hot day and EE decided not to wear protective gloves.
Employee was opening a piece of limestone using a 12 lb hammer & chisel. After striking the chisel/stone, employee saw blood on their shirt and realized they had a puncture wound to lower left quadrant of the stomach area. Foreign Body left in puncture wound

Miner was bending over using a Hammer & Chisel to trim/open a piece of limestone. As EE was striking the chisel, a piece of the chisel flew into EE's lower leg leaving a FB inside the puncture wound.

Miner was walking around near work site area and stated he stripped over a piece of stone - left knee pain/strain.

Miner reported on 3/24/15 that he had been seen in a clinic on 1/26/15 due to having pain in the groin area since Mid December. He was working throughout winter as weather would permit, while being on partial layoff. He was informed he had a hernia. He remained working on & off lifting no more than 10 lbs.
