Struck by flying object

Miner was splitting limestone slab with a chisel while striking it with 10 lb. hammer. A small piece of the chisels outer edge broke, causing foreign body to lacerate and get embedded inside miners' arm.

Rock was wedged in the conveyor belt. The on/off switch was bumped to try to free it. Did not say ""go"", EE said ""No"". When the conveyor bumped forward the rock bar was in the tail pully resulting in foreman being struck in the chest with the bar throwing them 6-7' in air and landing on the ramp.

Service Technician was performing maintenance on an impact crusher, rotating the impact blow bars. While handling one of the bars EE lost grip and it slipped out of EE's hand. A sharp edge on the blow bar cut the palm of EE's hand, requiring 5 stitches to close the wound. It was a very hot day and EE decided not to wear protective gloves.

Miner was using a 10 lb hammer and a chisel swinging at an angle to open the seam on a piece of limestone. Miner missed the chisel, causing a blunt force contusion to the top of their left hand resulting in a traumatic hematoma.

Miner was doing maintenance on one of the conveyors down on the stone crushing plant. EE was standing on the ground reaching slightly up to push over the portable electrical box, and as ee stretched and pushed off with left leg, ee heard a pop and immediately had shooting pain on back side of left ankle.

Miner was feeding the saw table with a block of stone. As ee lifted and set the stone onto the table ee caught the edge of pinky finger against the metal table and the force of the stone causing rupture to finger

Miner was lifting a piece of limestone approx. 70lbs with the help of a co-worker. He felt a strain on his right side and couldn't finish lifting.

Miner was lifting a piece of limestone, approx. 75 lbs from knee level while turning to place it onto a pallet. He heard a ""crack"" and immediately felt his muscles on the right side of his back tighten up.

Miner was catching thin stone inside the saw shop and was stacking it onto a wood pallet. He had one piece in each hand and as he turned to stack them onto a pallet, he tripped on the corner of the pallet and fell into it scraping his knees, hitting his chest, and spraining his right wrist

Miner was operating a Caterpillar 988 Front End Loader driving between stockpile & crusher. Quarry roads and floor are maintained by him to his liking so they are kept in quite decent shape. Possibly was operating too fast for conditions causing bouncing and/or jarring causing a low back injury.

The plate slipped out of his right hand, and fell landing onto his left hand/fingers which were still below in the area of the stone chip & debris causing crush injury to left index finger.

Miner reported on 3/24/15 that he had been seen in a clinic on 1/26/15 due to having pain in the groin area since Mid December. He was working throughout winter as weather would permit, while being on partial layoff. He was informed he had a hernia. He remained working on & off lifting no more than 10 lbs.
