Showing all 6Employee operating track hoe, went to disembark from track hoe, forgot placement of tracks, stepped out of cab and fell to the ground.
Dismounting CAT 400 End-dump. The individual was climb out of the cab. His feet entangled on step and hand slipped off door, fell backwards and landed on back and right elbow.
The employee states he reached under the guard to remove a piece of leggen on a 60" belt with his right hand while the conveyor belt was moving. The moving belt caught his right hand forcing the hand into the roller of the conveyor belt, cutting off the first joint of his right index finger. All guards were in place.
ROCK FELL FROM WALL AND STRUCK FRONT OF TRACK HOE, BENDING A PIECE OF METAL NEAR OR LOCATED NEAR THE FEET AREA OF THE CAB. THIS METAL BENT STRIKING THE LEFT LEG.
