Showing all 10Employee became ill complaining of chest pain and dizziness. 911 was called, he was transported to Lampasas Hospital. He was diagnosed with ruptured ulcer on colon.
Employee was standing on wet rock, slipped down, right arm stopped fall.
Wet, muddy ground, employee was stepping up on to the crusher to work on the crusher, his foot slipped and he hit the left side of his head causing injury to left side of face and left eye. He did refuse medical care, but changed his mind, and was taken to dr around 9:10 am.
Employee was using 4" grinder on Excavator EX-3 in a tight area when he slipped and the grinder cut his forehead requiring six stiches.
While climbing out of excavator, employee stepped off the tracks and onto the ground. He then felt a sharp pain run down his back.
A gust of wind blew dust in his eye.
Employee was assisting three co-workers change a bearing on the primary shaker. As they installed the counter weight, which was being held in place by a come-along, which in turned slipped causing employee to get his hand caught. Injury turned out to be minor contusion.
While working on shaker HR2 employee removed the bearing housing and it fell and crushed his finger between the bearing housing and wall.
WHILE WORKING ON SHAFT, A METAL GUARD FELL ON HIS FINGER CUTTING IT.
