Caught in, under or between a moving and a stationary object

Employee was prying out some fender bolts in the process of removing the bolts the pry bar slipped causing the right index finger getting pinned between the pry bar and equipment frame.

The employee overreached a guard to spray a head-pulley with brake cleaner to prevent the conveyor belt from slipping. As employee was spraying the head-pulley the pulley became in contact with the spray can, causing the can to get pulled in. In the process employee fractured right thumb.

EE strained his right knee dismounting a front-end loader. He did not report his injury immediately. Two days later he requested to go to the doctor. He did not improve as expected and was put on restricted duty.

Struck by flying object

Employee was installing a bearing on a conveyor head pulley shaft, he slid the bearing onto the shaft and was using a chisel and hammer to adjust the bearing. While hammering on chisel the employee felt something strike his left wrist, when he examined his wrist he could see a piece of metal embedded in his wrist.
