Showing all 9Worker was moving a smaller rock in the jaw to access and remove an over-sized rock. Upon grasping the smaller rock, worker's footing slipped allowing the smaller rock to shift, pinching worker's fingertip between the smaller rock and jaw side wall.
Employee was using a 1/2" drive breaker bar and socket to remove a nut from a bolt attaching a hopper wing extension that was in a bind with spring like tension. When the nut came off the bolt stored energy was released through the hopper wing sending the socket end of the breaker bar into the employee's face.
Turned and hopped from step/rail approximately 2 feet high and back went out.
The employee had on safety glasses and a chip flew into his eye. He was grinding something.
EE CLIMBED OVER HANDRAIL ON CRUSHER. HE SLIPPED, STUMBLED, THEN JUMPED, LANDING ON RIGHT ARM. RESULTED IN ELBOW FRACTURE.
DUMP TRUCK TIPPED OVER EDGE OF STOCKPILE LANDING ON ITS SIDE. DRIVER HIT HEAD CAUSING SCALP CONTUSION, MILD CERVICAL STRAIN.
SHOULDER STRAINED WHILE ATTEMPTING TO CLEAR JAM IN CONE. SAW DR ON 4-7-03. LIMITED LIFTING WITH LEFT ARM FOR 7 DAYS.
SHOULDER WAS STRAINED WHILE WRENCHING ON A BOLT ON THE CONE. DR. PRESCRIBED PRESCRIPTION ANTI-INFLAMMATORY & LIGHT WORK, NO LIFTING OVER TEN POUNDS FOR 7 DAYS.
