EE was opening railcar covers for loading. As EE traveled length of the RR car, lanyard began to extend downward. Lanyard got stuck at attachment point of cable. Lanyard began to deploy. Once fully deployed, lanyard became taut, suddenly stopping EE as he traveled from one RR car to the next, causing EE to lose balance and fall (fell to pavement next to railcars).

The employee was helping to remove the 1935 drag chain. Not realizing the chain was caught on a piece of angle iron, the employee looked into the opening on the west end of the Polycom to see what the chain was catching on. As the employee did this he leaned his hand on the chain and it came loose, striking his right thumb.

The employee was walking towards the east exit door of the yard break room area when he tripped on a pump sitting on the floor and twisted his right knee. Per call to company on 1-6-04, he fell against the pump, he did not fall to the floor. coder 12 This has become a reportable incident as of the doctor visit of 12/7/2004 at which time fluid was drained from employee's knee.

EE was checking the 903 belt as it was off track & having problems with the north tail pulley bearing. When he put his hand to the bottom of the guard to feel for heat, the next thing he knew something grabbed his finger. The EE thinks the zero speed paddle caught his fingerr & pinched it against the guard, causing a laceration to the middle finger of his right hand.

The employee was using the Fast Vac to clean up piles of coal on the 2nd floor of the Coal Mill. The employee allegedly received a static shock; dropped the hose and went to backup. When the employee backed up he allegedly tripped on the Fast Vac hose and fell. He allegedly twisted his right leg and felt pain in his right knee.

WHILE REMOVING THE WRENCH THE BACK FLOW PRESSURE FROM THE C OOLER CAUSED THE FAN TO ROTATE AND THE WRENCH PINCHED THE EE'S FINGER BETWEEN THE WRENCH HANDLE AND FAN BASE.

FORCE WAS APPLIED TO THE PULLER VIA ELECTRIC PUMP. THE HOUSING FRACTURED & THE PULLER BROKE LOOSE UNDER TENSION. THE RELEASE OF PRESSURE CAUSED THE PULLER TO FLY IN AN ARCH, STRIKING THE EE IN THE RIGHT SHIN (TIBIA).
