Mechanic was connecting a conveyor to a screener. He was on the conveyor when it started up and he fell off.
Standing in yard, called mechanic stating he thought he was having a heart attack. EMS summined, stopped breathing, revived in ambulance. Stopped again enroute to hospitl. Hospital attempts to revive unsuccessful. Pronounced dead of cardiac arrest. Submitted - Pending MSHA chargability determination.
Turned to pull heavy hose, felt pain in lower back
Reported right shoulder, arm and elbow hurt too much to work. Hurt previously, he continued to work; not disabling. On vacation difficult to move. Off from work, will receive therapy. Recalled 12/2/2005 at 07:00, frosty morning, climbing a fixed ladder at cone, slipped, grabbing on and held with his arms. He pulled himself up and he continued to work.
Steep incline down to quarry bench; loose material and broken rock on surface of incline (ramp). Employee slipped and fell while carrying packaged explosives to work area.
Groundsman was bending and picking up a metal safety guard to place on scale belt conveyor, states strain hurt back.
Climbed out of chute and jumped down about three feet and slipped on frozen ground, twisting his left ankle.
Rock jammed in cone; while removing rock, employee twisted right wrist

EMPLOYEE WAS ATTEMPTING TO DISLODGE A ROCK IN CONE CRUSHER THAT CLOGGED CONE. HE STUCK HIS LEFT HAND INTO PILE. ROCK BOUNCED OUT AND HIT HIS MIDDLE FINGER.
EMPLOYEE WAS ATTEMPTING TO UNJAM SURGE BIN BY HAND AND A LARGE ROCK FELL FROM TOP OF RIB, CAUGHTEE'S LEFT LITTLE FINGER CRUSHING IT. EE WAS WEARING LEATHER GLOVES AT TIME OF ACCIDENT.
EMPLOYEE WAS ATTEMPTING TO UNCLOG CHUTE ON CONVEYOR WHEN A PIECE OF ROCK FELL OVER TOP OF CHUTE AND LANDED ON RIGHT INDEX FINGER CRUSHING IT AGAINST THE METAL SKIRT BOARD OF THE CHUTE, CAUSING A SEVERE LACERATION TO RIGHT INDEX FINGER.
DRIVER WENT TO CHECK TAILGATE ON TRUCK BEFORE DELIVERING A LOAD AND GOT FINGERS CAUGHT IN THE TAILGATE IN THE PROCESS.
