Caught in, under or between running or meshing objects

Employee was changing the screen out on a screener deck, which was locked out. The adjacent conveyor was still running, however. As the employee lifted the screen, the screen was pulled into the running conveyor, pinching left index and middle fingers against the running conveyor belt. Employee suffered a laceration and tendon damage to left middle finger.

EE WAS LEAVING OFFIC TRAILER, SHE SAID SHE WAS SAYING GOODBYE AND MISSED THE FIRST STEP OFF OF LANDING. SHE LANDED ON BOTH KNEES WHICH RESULTED IN BOTH KNEES BEING BRUISED.
