Employee was walking from bay 7 to bay 12 to help GR573 cutting edges, while walking across yard they stepped in a hole felt right knee pain. They became a lost time on 04/15/2026 due to right knee sprain.
Employee was walking through warehouse when their R knee strained due to pothole in walkway.
Employee was crawling out of CR02 chute after using a jack hammer to break rock plugging chute. Employee was pushing the jackhammer in front of EE. it caught a rock causing the bit end to pop up towards the employee, striking EE in the teeth. resulting in a broken tooth.
Operator was changing out a bit; they stepped off of the deck and when their left foot made contact with the lower deck of the drill, they felt their left ankle roll to the outside and felt sharp pain in the left knee.
Employee was diagnosed with a standard threshold shift by Dr on 03/19/2026, this hearing loss is consistent with noise induced loss in the left ear
EE was stepping off the Haul Truck ladder onto uneven ground, when EE felt and heard a loud pop, and states EE felt instant pain.
Operator was stepping off of pickup truck and right food stepped in a hole and rolled right ankle, resulting in a sprain. restricted duty began on 12/11/2025
The driver was completing a pre-trip inspection on the trailer. Driver was crouched down and stood up, thinking they had enough clearance, and hit their head on the trailer. Driver was wearing a hard hat, which compressed their neck.
Employee was screwing into plastic reputs. Due to prescribed antibiotics employee developed Photophobia.

While installing a shock brushing in truck 143's lower A arm, EE went to knock it in with a hammer, EE missed striking EE's Right thumb. received 6 sutures
Employee picked up a plywood bulkhead for a concrete form and when they turned to put it in place it caused a strain in their back.
As the employee was ascending the ladder, felt a pop in left shoulder
Employee was diagnosed with a standard threshold shift by a doctor on 09/18/2025. This hearing loss is consistent with noise induced in the left and right ear.
When they placed a hand up to stop the door/panel from slamming, their left ring finger went into the hinge section. When the door closed it caused injury.
Employee was picking up a 5-gallon bucket of ore one handed when they felt a pop and pain in right shoulder
When checking spigots at tailings walking, they lost footing and fell into a ravine/ditch onto their chest.

Employee was feeding a cathode through the roller when the end of the cathode grabbed the tip of their left middle finger and pulled their hand into the guard. Resulted in avulsion of left middle finger tip.
Employee was installing a new bearing using a 10lb sledgehammer when they hit their foot just past the steel toe. resulting in the fracture of the minimus pedis (Little toe)
Employee was performing EE's area work inspection when EE slipped and heard a pop in the right knee. Resulted in aggravation of old knee injury.
Employee was walking in the tie down area around the water truck, when they slipped and landed on their right wrist, resulting in a fracture. Employee was placed on restricted duty.
Employee was dismounting from equipment. While activating the ladder to exit, they began descending before the ladder had fully reached the ground. As a result, the ladder accelerated rapidly. This sudden movement caused employee to lose balance and fall sustaining injuries.
Employee lost EE's footing and stumbled causing EE to lacerate EE's head on a steel window frame of a connex box. Employee received 6 Staples
On 05/08/25 employee struck in arm by foreign object did not report the incident. The following week the incident was reported. Foreign body removed. 4 stitches received. Notified 05/27/2025 that the injury was accepted as an industrial injury.
Employee was walking in a dim area when EE tripped and fell over forks from a forklift, resulting in right shoulder pain. On 4/22/2025, employee was placed on Restrictive Duty
Injured employee was spinning a pulley sheave by hand to remove an impeller and caught two of EE's left fingers in the belt. EE's left middle finger was badly lacerated and EE's left ring finger sustained a fracture.
While dismounting a trailer, Temp employee encountered a loose step causing EE to fall 3 feet onto ground scratch on left elbow and left foot pain resulted in left foot fracture.
Employee was drilling a hole in a new disconnect box. The drill bit got caught and pulled the box into employee's hand. Caused a cut and tuft fracture to right index finger.
Assigned to change out a steering pump and in process used a come-along to hoist up pump into place, felt a pop in left bicep
Employee was washing under the 219 ball mill. Employee crossed over the 212 cyclone feed pump discharge line by stepping on the angle iron brace supporting the pipe. While stepping over, employee slipped and rolled an ankle. Employee was placed on restricted duty 02/18/2025.

Employee was using 1 inch impact gun on bolt under truck. Bolt was not coming off causing the employee to adjust position. As employee was adjusting the impact gun it bounced back hitting employees face causing a small chip top of *R* tooth. 4/10/25 EE did elect to repair tooth, dental work completed
Employee noticed idler that was frozen and attempted to free this idler up using a pry bar, the prybar was grabbed by the running Conveyor and pulled in between the trougher idler and the Conveyor belt, this caused the base of the prybar to smash the employee's left hand between the frame and the Prybar.
Cutting tape on middle slim, hit a tough spot, applied more pressure hand slipped and cut left knee, 7 stitches received.
While conducting a team lift of a front end of a light plant employee got their finger pinched in between the tongue and the ground, sustaining a fracture/break to the tip right index finger. Employee was evaluated at the Occupational clinic and allowed to return and resume normal work activities, once finger was cleaned and splinted.
Employee was at the media charging area heading down from a fixed ladder on the east side of the station. Approaching EE's last step, EE slipped from the ladder and rolled EE's right ankle after landing on the kick plate/grading foot guarding.
Employee hooked toe under next to last step on stairs and fell on knees causing abrasions and swelling. Swelling and soreness in left knee is impairing mobility.

The employee was working on changing out EGR coolers on the engine of the MM485 water truck. In the process of removing the passenger side EGR cooler employee had arms out and had good control over the cooler (about 40 lbs). As EE was bringing it down, EE heard and felt a pop in their left forearm/ elbow area.
Employee struck a pin with a hammer and a small fragment of metal hit EE in the face causing a piece of metal to embed into the skin.

Upon pulling the cord, the cord jerked EE forward causing a popping sensation in EE's right arm resulting in a right wrist strain.
The employee was walking when they tripped over rocks on the ground and caught themselves by holding onto a tanker vehicle, injuring their right wrist. The driver was evaluated by a medical professional and released to full duty. On 11/11/24, a surgery was performed to address injury at which point the driver was taken off work for surgery and recovery time.
