NOTE: HAND INJURY OF 2/20/03 WAS NOT HEALING AS EXPECTED. DECISION WAS MADE TO OPERATE, RESULTING IN EE BEING OFF WORK FROM 8/25/03 TILL 10/13/03 RETURN TO WORK DATE.
GRINDING W/ HAND GRINDER ON 966 TOOTH REPLACEMENT. PIECE OF METAL WENT IN EYE, PIECE FELL IN BETWEEN SAFETY GLASSES AND EYE THE BOUNCED BACK INTO EYE.
