Employee fainted for approx 1 minute. Regained consciousness and stated he improperly taken blood pressure medication, was transported by rescue service for Dr evaluation. Due to heart condition, pacemaker installed later by personal physician, 37 days off.

It had snowed in Georgia on 2/12. We shut down at 1700 on 2/12. Plant maintenance, pit and production came in at 0800 the morning of 2/13 to clear snow from belts, chutes and screens so that we would be able to produce that afternoon and night. Employee said he slipped a few times while hurrying around the plant. He reported a back injury on 2/16.

Technician was hammering with sledge hammer and hit his left hand pinky (lower hand on hammer handle). Finger on fixed adjustment bolt which broke the bone in the finger.

Employee was not on shift waiting to clock in at the time clock, when he started started experiencing chest pains and shortness of breath. EMT's were called and employee was transported to Hospital. He suffered a massive heart attack while at the hospital during examination. Aggregates USA DOL does not believe accident injury illness citation 6598897

Tech. was torquing bolts with hyd. wrench when it became wedged. While he was trying to get the wrench head loose, the on/off switch stuck. When the wrench head came loose it cut middle finger on right hand.

Employee was cutting plastic tie wrap with pocket knife when the knife slipped and cut his left hand on the side of his forefinger. Cut required sutures and employee was able to return to work with no restrictions.

Employees were removing a shaker bearing when the hammer used to drive the bearing loose from the housing chipped off and struck an employee just in front of his right ear that required sutures and prescription medication.

Employee was installing a skirt board on a conveyor when the pry bar he was using slipped and pinched his finger between the bar and an I-Beam that resulted in a cut to his right index finger that required sutures. Employee was released to return to work on the same day.

Employee was using high pressure water hose for clean up when an unknown object flew up and struck him on his cheek bone just under the right eye that caused a small cut. Employee was taken for medical observation and recieved 3 stitches and a tetnus shot. Employee returned to work with no restrictions.

Employee was getting into loader seat holding onto door. A strong wind caught door, closing it suddenly causing employee to strike his head on window latch inside cab of loader causing laceration to head.

Employee was cleaning around tail pulley under plant and a rock fell and struck him on his lower back. Employee was sent for medical treatment that consisted of prescription medication for bruising and was released to work to perform normal job duties.

Employee was using a empty 5 gallon bucket to dip water and pour into a pump for priming. He had filled the bucket approximately 1/2 full and when he lifted the bucket up to pour it he felt a twinge in his lower back. Employee was seen by his doctor and X-rays were taken that were negative. Employee was given prescription medication with no work restrictions applied.

EE MINER STEPPED OVER DISCHARGE PIPE ON DREDGE. SURFACE WAS SET DUE TO LEAK THAT WAS BEING REPAIRED. MINER TRIPPED ON WET SURFACE, FALLING BACKWARDS STRIKING LOWER BACK ON PIPE, CAUSING CONTUSION TO LOWER BACK.
