EE was tracking Conveyor belt, and saw an Idler not rotating. EE used their hands to try to break the Idler free, The roller starting rolling which resulted in them being pulled into the Idler. their left arm was pulled between the Idler and belt.

Over-exertion in lifting objects

On 9/14/17, the injured employee was attempting to connect a pipe to a pump in the pit when employee felt pain in left shoulder. Employee reported the accident at that time but did not seek medical treatment. Injury became reportable on 11/20/17 when the employee went to the doctor and was given restrictions.

Struck by falling object

While moving a pallet of parts in the bed of the pick-up truck, the pallet fell and caught the employees fingers between the pallet and the parts. This resulted in a laceration to the ring and middle fingers on the right hand, requiring 5 stitches.

Fall from machine

Injured employee sprained his left ankle as he jumped down from the CAT 775D haul truck to extinguish a fire. After the original treatment of the injury, the employee required an additional medical procedure to treat swelling/blistering that had developed in the area that was splinted. The fire was extinguished in less than 10 minutes.

Employee was working in chute cutting a 3"" X 30"" piece of metal when the cut metal dislodged and ricocheted off the opposite wall and struck the top of the employee's right foot resulting in a laceration requiring (6) stitches.

TECHNICIAN HAD TO UTILIZE MANLIFT TO PERFORM MAINENANCE ON SCALE READOUT. THEY FINISHED & MOVED THE MANLIFT TO AN OUTSIDE AREA. WHEN THEY ATTEMPTED TO STOP, THE UNIT SHUT OFF ABRUPTLY WHICH FO RCED THEM ONTO THE SAFETY RAIL OF THE MACHINE & BRUISED THEIR RIGHT SIDE.

EE WAS HELPING TO MOVE A SUCTION HOSE WITH A ROPE WHEN AFTERWARDS HE WENT TO THROW THE ROPE TO HIS SUPERVISOR HE SLIPPED & FELL, HURTING HIS LEFT KNEE.

ON 11-29-01 BETWEEN 12-2PM EE WHO IS THE YARD LOADER FOR A 980F, WAS ABOUT TO LOAD A TRUCK (CONTRACTOR) WHEN HE DUG HIS BLADE OR BUCKET ANDSTRUCK A ROCK IN THE STOCKPILE. EE REPORTED HE WAS ""JARRED"" INSIDE HIS CAB AND THUS HURT HIS NECK (STRAINED). HE WAS LOADING #1 RIP RAP.
