Employee claims that while dismantling a water pump in shop facilities his right hand slipped under the pump and table causing a "pinch" on his right thumb. He continued to work the rest of the day without incident until later complained about his hand.

After working for our company for only 3 months, the employee claims lower back problems from work done several weeks ago.

EE was unconscious approx. 25 mins.; regained consciousness prior to being taken to hospital. EE was wearing hard hat at time of accident. Diagnosis: concussion.

The ee was trying to disjoint a tube in the conveyer. The tube loosened up and caught his right hand, affected his little finger.

Waiting to be loaded the ee decided to get off his truck and while walking outside and around the pit area he tripped hurting his right elbow, hand and back.

THE EE WAS CLEANING THE CONVEYOR WALKWAY AREA WHEN A STONE BOUNCED INTO HIS RIGHT HAND, CUTTING HIS RING FINGER.

THE EMPLOYEE WAS WALKING UNDER THE CONVEYOR WHEN A 3 TO 4 INCH STONE APPROX FELL ON HIS HEAD. THE EMPLOYEE HAD SECURITY HELMET BUT DIE TO THE IMPACT AND SIZE OF BOULDER EMPLOYEE SUFFERED A CUT TO HIS HEAD AND DIZZYNESS.

The ee was cleaning the primary crusher balcony area and dirt fell into his left eye. This caused irritation in the eye, but he continued working. The next day his eye was swollen, and could not work.

THE CONTROL ROOM OPERATOR CALLED EE CONCERNING A PROBLEM WITH THE TERTIARY CRUSHER MAIN BREAKER. EE STARTED THE TROUBLESHOOTING BY CHECKING VOLTAGE ON THE TERTIARY MAIN BREAKER WITH AN ANALOG VOLTMETER; AN EXPLOSIVE SHORT CIRCUIT CAUSED EEBURNS ON BOTH HANDS AND FACE. CAUSE OF ACCIDENTCOULD HAVE BEEN THE WRONG SETTING ON THE ANALOG VOLTMETER WHEN TAKING THE READING.

WHEN HE WENT TO GET HIS PAYROLL CHECK AT THE OFFICE HIS FEET SLIPPED AND GOT AN INJURY AT THE ANKLE AREA. AT THE MOMENT OF THIS ACCIDENT HE WASN'T AT WORKING HOURS.

HE WAS WORKING ON THE MAINTENANCE OF A MACHINERY CHAIN AND THE GLOVE IN THE RIGHT HAND WAS STUCK OR CAUGHT IN IT CAUSING HIM AN INJURY ON A FINGER OF THE RIGHT HAND.

THIS EMPLOYEE WAS DOING MAINTENANCE TO MACHINERY PART IN THE MAINTENANCE AREA, AND HE WASN'T USING SECURITY EYE GLASSES WHEN CLEANING THIS PART. A PIECE OF THE PART HIT HIM ON A EYE.

THE EE WAS WORKING ON THE MAINTENANCE OF A CONVEYOR BELT, HE DIDN'T STOP THE MACHINE TO CLEAN IT, THEN THE MACHINE HURT A FINGER OF THIS EE.

WHILE WORKING ON THE INSTALLATION OF THE WEIGHT BALANCE HE STUCK A NAIL UNDER HIS FOOT THAT WENTIN THROUGH HIS SHOE. HE HAS TO STAY OUT OF WORK FOR UNKNOWN DAYS.

UNCOVERING VALVE PINCHED FINGER ON RIGHT HAND.
