The employee was cleaning the primary when his right foot slipped under the main rotor.
Employee claims fell from loader steps two week ago. He claims lower back pain. He never reported his fall to any of his supervisor until now.
The EE while cleaning drum pulley with rebar while operating. The rebar was caught by the drum pulley injuring index finger.
While cleaning the muel pump area, a pare part of the mud pump was secured by a chain, the EE decided to push it aside when it dropped in his right hand.
Employee alleges he sprained ankle while descending from stairs leading to the primary area.

While inspecting the skid loader after operating, the EE resulted with minor burns on hand and chest when he removed the radiator cap.

EE was cleaning between conveyor 20 and conveyor 7 area. A stone fell (from operating conveyor approx. 6"" above EE's head), hitting EE on back of head, right side. EE was unconscious approx. 25 mins.; regained consciousness prior to being taken to hospital. EE was wearing hard hat at time of accident. Diagnosis: concussion.
The ee was trying to disjoint a tube in the conveyer. The tube loosened up and caught his right hand, affected his little finger.

Waiting to be loaded the ee decided to get off his truck and while walking outside and around the pit area he tripped hurting his right elbow, hand and back.
THE EE WAS CLEANING THE CONVEYOR WALKWAY AREA WHEN A STONE BOUNCED INTO HIS RIGHT HAND, CUTTING HIS RING FINGER.
THE EMPLOYEE WAS WALKING UNDER THE CONVEYOR WHEN A 3 TO 4 INCH STONE APPROX FELL ON HIS HEAD. THE EMPLOYEE HAD SECURITY HELMET BUT DIE TO THE IMPACT AND SIZE OF BOULDER EMPLOYEE SUFFERED A CUT TO HIS HEAD AND DIZZYNESS.
The ee was cleaning the primary crusher balcony area and dirt fell into his left eye. This caused irritation in the eye, but he continued working. The next day his eye was swollen, and could not work.
THE CONTROL ROOM OPERATOR CALLED EE CONCERNING A PROBLEM WITH THE TERTIARY CRUSHER MAIN BREAKER. EE STARTED THE TROUBLESHOOTING BY CHECKING VOLTAGE ON THE TERTIARY MAIN BREAKER WITH AN ANALOG VOLTMETER; AN EXPLOSIVE SHORT CIRCUIT CAUSED EEBURNS ON BOTH HANDS AND FACE. CAUSE OF ACCIDENTCOULD HAVE BEEN THE WRONG SETTING ON THE ANALOG VOLTMETER WHEN TAKING THE READING.
