The ee was cleaning the primary crusher balcony area and dirt fell into his left eye. This caused irritation in the eye, but he continued working. The next day his eye was swollen, and could not work.
THE CONTROL ROOM OPERATOR CALLED EE CONCERNING A PROBLEM WITH THE TERTIARY CRUSHER MAIN BREAKER. EE STARTED THE TROUBLESHOOTING BY CHECKING VOLTAGE ON THE TERTIARY MAIN BREAKER WITH AN ANALOG VOLTMETER; AN EXPLOSIVE SHORT CIRCUIT CAUSED EEBURNS ON BOTH HANDS AND FACE. CAUSE OF ACCIDENTCOULD HAVE BEEN THE WRONG SETTING ON THE ANALOG VOLTMETER WHEN TAKING THE READING.
WHEN HE WENT TO GET HIS PAYROLL CHECK AT THE OFFICE HIS FEET SLIPPED AND GOT AN INJURY AT THE ANKLE AREA. AT THE MOMENT OF THIS ACCIDENT HE WASN'T AT WORKING HOURS.

THE EE WAS WORKING ON THE MAINTENANCE OF A CONVEYOR BELT, HE DIDN'T STOP THE MACHINE TO CLEAN IT, THEN THE MACHINE HURT A FINGER OF THIS EE.
WHILE WORKING ON THE INSTALLATION OF THE WEIGHT BALANCE HE STUCK A NAIL UNDER HIS FOOT THAT WENTIN THROUGH HIS SHOE. HE HAS TO STAY OUT OF WORK FOR UNKNOWN DAYS.
