Employee was attempting to install a coiled spring into a collar that holds a bit of a small jackhammer. EE almost had the spring inserted, but was having a problem with the spring going in all the way. EE removed right hand glove to push the spring into place. The spring embedded into finger.

Employee was shoveling off fly ash from the top of 1 and 2 dust tank. The employee alleged that something popped in their lower back when they lifted a shovel of material.

The injured employee and another employee was erecting a 30 foot conveyor into place. The conveyor was suspended with chain and cable hoist. The conveyor needed to shift to the left a couple of inches. The injured employee had thumb in the area where IE believed it was hung up. The conveyor unexpectedly shifted forward pinching IE's thumb between the chute and conveyor.

Employee was carrying a bucket to gather some rock from a conveyor for his hourly sample. He stepped down into the discharge belt conveyor area. The employee didn't look where he was stepping. He stepped on a clinker causing him to twist his ankle.

The injured an another employee was replacing the bearing on the drag chain sprocket. The sprocket was suspended, as they was positioning the sprocket into place. The sprocket moved forward wedging the injured employee's thumb between the sprocket and drag chain compartment.

Employee went to the warehouse to get a wire cloth screen. He pulled the screen from the rack. He leaned the screen against the trailer. He was lifting the screen lost his grip, the screen was falling toward him. He reached quickly to catch the screen. He felt a pop slight burning between his shoulders.

Employee said that he exited the #1 and #2 Kilns control room to the # 1 kilns firing deck. He felt a twitch or pinching feeling to his left knee. He returned to the control room. When he would put pressure to the heel of his left leg, he felt sharp pains to his left knee.

Employee hooked the crane cable block into the lifting eye of the conveyor belt carriage. When the crane operator began cabling up, employee seen the cable block drifting toward a 55 gallon oil drum. He pushed on the block with his right hand. The outer cable caught his right thumb, pulling it into the cable block.

Employee was using a parts puller to pull a coupling hub off a shaft, he said it was tight. He was using an adjustable wrench to turn the treaded shaft. He was using repetitive motion to remove the hub. After removing the hub, he turned and felt a sharp pain to his neck.

The employee exited his vehicle and was walking to his work area. It had been snowing for several hours. He stopped to make contact with the front-end loader operator working in that area. He took a couple more steps lost his balance and fell.

Employee stepped out of the 3&4 control room onto the #4 side kiln firing deck. Upon stepping, some grit blew into his eye. He stepped back into the control room and removed his safety glasses, then rubbed his eye. The grit scratched his eye when he rubbed it.

The employee was using a hammer and punch to remove the cut bolts from the connecting ring on the ID fan stack. A piece of the punch chipped off the top cutting his hand. At the time of the injury, the employee didn't realize that a piece of the chipped punch was in his hand!

# 7 Rotary kiln was taking down with a problem to the drag chain. The injured employee was washing material away from the drag chains drain chute. The maintenance man dropped his flashlight, he ask the injured employee to toss it back to him. the light hit the wall fell onto the conveyor belt. The injured employee reached for the light, tripped on a plate that divides the tunnel.

Employee was cleaning up after a maintenance job on the # 6 baghouse. He had bent down to grab the welding and ground leads and a cutting torch. As he proceeded to move forward and began to stand up, he rammed his head into an air pressure receiver pipe that was in clear view.

EE and another ee where changing the oxygen bottle on a portable welder trailer. The other ee set his end of the tank onto the trailer. As ee lifted his end to stand the tank upright, the other end slipped of the trailer, jerking ee left shoulder.

Employee was walking toward the silo talking to the loader operator on a hand held radio. He stepped on a rock, twisted his ankle. He caught himself before he hit the ground. He stood up real quick, he felt something pop in his lower back.

Employee was exiting the cab of the water truck. He hold the steering wheel with his left hand and the grab iron with his right, with both feet on the top step. He placed his left foot on the lower step, as he lifted his right foot his left foot slipped of the step, allowing his right foot to be caught between the cab and first step. With ee falling backwards to the ground.

A couple weeks ago he had a crick in his neck. It went away but stayed irritated up until 08/14/12, this is when he reported to his supervisor that his neck was pain. The loader pins are worn causing the bucket to jolt his neck back and forth. The loader was taking out service on the 15th an the bucket pins replaced.

The employee was helping another employee carry a woven wire screen. As they were setting the screen down, the edge of the screen rubbed the injured employee's elbow cutting him approximately four inches.

EE was washing down some material when a rock, approximately 12" x 8", slid into the EE's left foot. At the time, the EE said it was fine. On 9/14/2011, the EE complained of pain in his foot, he went to the doctor and the doctor said he had a bruise. The doctor told him to prop his foot up occasionally while at work, restricting his duty, thereby making the incident reportable.

The EE was welding inside the loader bucket, using a LN25 wire welder. That Friday afternoon his eyes were sore, by Saturday evening his eyes became worse and began to swell.

Employee's were working inside the #2 Kiln cooler system. The employee that was injured had his arm over one of the cooler flight, he was tightening the bolts that holds the cooler flight in place. Another employee placed a cooler plate onto the flight and pushed in into employee's left hand.

The employee was replacing the maintenance service door on the #1 coal mill, as he was tightening one of the treaded bolts, it broke causing the employee to suddenly lose his balance. He regained his balance a short time later he felt a sharp pain to his lower back (Lumbar area).

Employee clocked in at 7:00 a.m., he had been feeling sick since 3:30 a.m. that morning. He came to work anyway to learn he wasn't feeling any better. Another employee helped him to the office where he described to us that he was having chest pains and difficulty breathing. We called 911 for assistance.

The EE had gathered a five gallon bucket of stalite material and emptied a five gallon bucket of coal. Minutes later he was walking to the EE's break room and felt a pain to his chest area and he developed a cold sweat. He thought he was having a heart attack.

Employee was injured when two other ee's were using jack hammer to chip away edge of hot clinker. Tje employees stopped working to take cool down break. One of the employees found the other employee unconscious away from the area where they had been working.

EE was walking to inspect the clinker tunnel conveyor. The area which he was walking was wet from a leaking water pump. The wet area had grown some algae and no one had noticed the slippery spot. The EE walked through this area and slipped and feel to the concrete pad.

EE's were working on the 6 and 8 "middle deck" of the three deck screening tower installing new screens. The EE that was injured was intending to use an electric impat wrench to tighten the 3/4 inch bolts that holds the screens in place. The other EE tossed an electrical extention cord to the EE which was injured when one end of the cord struck him at his upper lip.

EE was attempting to remove the anti-theft plastic band from a new socket wrench set. He began using a pair of pliers, which didn't work, so he used a utility knife instead to cut the plastic tabs. When he exerted pressure to the tab, part of the tab broke off allowing the force of the utility knife blade to hit his inner thigh, giving him a 1 inch laceration.

EE was unclogging a chute using a crowbar through a hole in the chute. When the rocks broke loose, the weight of the rock forced the crowbar down and the EE's hand got pinched between the crowbar and the chute. This caused a laceration to his thumb requiring stitches.

EE was using a Purox oxygen & accetylene cutting torch to cut an 8" I-Beam which was at a 45 degree angle & secured with a chain fall on 1 end & welded to another I-Beam on the other. It must have been under pressure because when beam was cut the chain fall slid down & allowed beam to drop Appx 3" & during this our employee's wrist became squeezed betwen the chain fall & beam.

On Monday 3/9/09 maintenance crew was doing repair work in one of our kilns. At this time no accident had occurred. On Tues morning the employee woke up with a crick to the right side of his neck. He alleged that the crick in his neck came from work being performed at the Carolina Stalite plant. He didn't report it until 3/16/09. Sent him to the dr. the same day.
