Caught in, under or between collapsing material or buildings

Miner dug out area between chute and belt. Miner looked into that area and a mass of material broke loose in chute and entrapped miner.

Struck against stationary object

He climbed up on the front of a shovel track to get into the cab and hit his head on a mirror bracket. He did not use the step mounted on the machine and came up under the bracket from the wrong direction. The ER doctor gave him 5 sutures and said he suffered a concussion.He was seen again by the doctor on 3/9/2010 and cleared to return to work on the 10th.

He felt hot, bed rested in scalehouse awhile, then went home. Later that night he went to the emergency room.

Struck by flying object

Rock slid off highwall, rock fragment came through window of excavator and hit operator on the left shin. He did not need medical treatment at the time. On 1/29/07 he complained of swelling and went to dr. He is being treated for cellulitis.

While working with Bobcat skid steer front loader operator to prepare a future stockpile area, victim's foot was caught between the bucket and arm of loader resulting in fracture to the big toe of his left foot.

INJURED WAS CLEARING BLOCKAGE FROM BOTTOM OF SQUARE VERTICAL CHUTE WHEN A ROCK FELL ON INJURED'S HAND AND CAUSED LACERATIONS. HOSPITAL EXAM WITH X RAYS INDICATED NO BROKEN BONES. INJURED LEFT LITTLE FINGER AND IS IN STEEL SPLINT. LACERATIONS REQUIRED NO STITCHES.

Fall onto or against objects

INJURED FELL ONTO METAL HOPPER AND BELT WHILE CLEARING BLOCKAGE AT THE BOTTOM OF A SQUARE VERTICAL STEEL CHUTE. NO EXTERNAL LACERATIONS OCCURRED AND HOSPITAL EXAM, WITH X-RAYS, INDICATED NO BR OKEN BONES. DIAGNOSIS WAS POSSIBLE LOWER BACK STRAIN.
