Showing all 5Loading scrap iron for offsite disposal. Big piece of metal hit EE's left ring finger. EE came in at 9:15 a.m. on 1/19/16, made a doctor's appointment at Seneca Family Practice. They said EE fractured left ring finger but will be able to return to work immediately.
Bent over to retrieve hard hat, stood up under sharp piece of screen box. Poked a hole in the top of head.
Walking to and stepping over water line with right foot first, then left foot, which caught the water line and caused the fall, hurting the right side and right bottom of right foot.
Coming down ladder struck right arm on I-beam. This accident occurred while pit was under construction, mining actually began 9/11/07.
