Employee was standing on a gravel loading ramp leaning over and looking into a hopper. Subject lost balance and fell between the ramp and the hopper. Subject struck an I-beam on the hopper with right thigh causing a laceration. Subject then drove to the ER where EE received 8 stitches. Subject plans on returning to work the next day.

While using an angle grinder, the disc came apart and struck the user in the chin area. User was grinding on metal on a conveyor when the incident occurred. User had recently replaced the disc and failed to put the guard back on. Also contributing to the injury is the fact that a face shield was not in use. User was taken to hospital and received medical treatment.

THE EMPLOYEE WAS SERVICING EQUIPMENT, STARTED IN THE MORNING AND HAD DRIVEN A SERVICE TRUCK DOWN INTO THE PIT. HE APPARENTLY HIT A ROCK OR A HOLE, SHAKING THE TRUCK AND FLIPPING A BARREL OF ANTI FREEZE OVER. HE ATTEMPTED TO PICK IT UP BY HIMSELF AND STRAINED HIS BACK.

EMPLOYEE WAS STANDING ON FRAME OF SCREEN TRYING TO UNDO A SHACKLE & CABLE THAT HAD BEEN USED TO LIFT THE FRAME TO ALLOW A TRUCK TO BACK UNDER THE SCREEN. NEEDING A WRENCH, HE PLACED HIS HAND ON THE FRAME OF THE SCREEN & JUMPED TO THE GROUND APPROX. 5'.

EMPLOYEE STEPPED OFF LOADER AND ADJUSTED SCREEN, HE WAS GETTING BACK ON LOADER AND THOUGHT DOOR WAS LATCHED, WAS NOT, WIND CAUGHT DOOR AND SLAMMED IT SHUT CATCHING LEFT HAND BETWEEN DOOR AND F RAME. SMASHED THREE KNUCKLES ON PINKY FINGER AND BOTTOM 2 KNUCKLES ON RING FINGER.
