Employee was taking down a piece of tubing. When it came down it struck the ladder EE was standing on causing EE to fall off the ladder and EE hit left arm on the tubing resulting in a strain to left shoulder.
Employee was installing the finisher steel in hole. The steel got stuck so EE pulled it out some then tried to shove it in the hole. When the steel got stuck again EE felt a pop in left shoulder and experienced pain. Employee started missing time on 6/24/16.

Employee had pulled walk thru roof bolter in #6 entry to bolt and set the T-bar on the roof bolter. The employee had grabbed a couple of bolts to lay in the bolt tray. EE placed the bolts down in the tray when a piece of undetected draw rock fell striking EE on the right ring finger resulting in a fracture to the end of the finger and required stitches.

Employee was helping pull the head shaft out of the feeder. The right side slid and came out sooner than the left side causing the right side to drop suddenly and the left side to rise up. The event pinched EE's wrist between the head shaft and the frame of the feeder resulting in a fracture to right wrist.

Employee was taking the PLM high voltage coupling loose from the section power center. The employee pulled the spanner wrench toward EE to loosen because of a metal guard that was on the back of the power center. When the coupling broke loose the end of the spanner wrench came back and struck EE in the mouth knocking EE's right front tooth out.
Employee alleges that while operating the scoop on the travelway towards the surface, EE hit EE's head on the canopy. The employee has missed work, however this alleged incident is under investigation. Due to the reporting deadline, this document is being submitted to comply with reporting criteria until full determination of actual injury and/or incident can be substantiated.
EE alleges while hanging an empty 2 inch waterline EE felt pain in EE's lower back. EE has not missed work, EE began working restricted on 4/12/16. Alleged incident is under investigation. Employee was tramming out by in a scoop to pick up rock dust when the scoop's front tire hit a hole in the bottom causing EE to bump EE's head on the canopy of the scoop. This resulted in a strain to EE's neck.
Employee was using a 1" air impact wrench to remove bolts from a cutting edge lip protector on a 993K CAT loader. EE was removing a bolt and it began to rotate. EE pushed up on the bolt and bumped the rotation of the air impact hoping the bolt would lock back into position but instead the bolt spun causing a laceration to EE's left index finger. This resulted in 17 stitches.

Employee was helping move the emergency sled when a timber fell off. EE bent over to put it back on the sled, in the process of replacing the timber the sled set down on EE's right foot. This resulted in a fracture to the 2nd and 3rd metatarsal in EE's right foot.
Employee was walking out by the miner when EE's legs got tangled up in the miner cable causing EE to twist EE's left knee as EE fell. This resulted in a fracture in EE's knee cap.
Employee states while throwing a garbage bag full of old boots and trash into the back of the pickup he felt pain and a burning sensation on the top of his right shoulder. He started physical therapy on 2/5/16. *Continued to work until surgery on 4/1/2016.

While working on a brattice, employee turned to get some cap wedges when he tripped over a plaster bucket. As he was falling his body made a twisting motion causing severe pain in his lower back continuing down to both of his feet.
Employee was operating CAT 785C when he went through a swag in the haul road causing his body to move side to side resulting in pain in his lower back.

Employee was installing an anchor bolt in the #5 heading. He was pushing the bolt to the top, stopped up pressure to turn the anchor, then continued pushing the bolt up. He caught his left middle finger between the wrench and the anchor resulting in a laceration requiring 3 stitches.
