Miner operator was taking 2nd cut out of #5 left entry break when a methane ignition occurred. The ignition came back across the miner burning the operator.

The shuttle car operator was loading behind miner in #5 left break when methane ignition occurred. The igniton came across the miner and back to the shuttle car striking the shuttle car operator on the left side of his face.

S/c operator making left turn in intersection. Oper side deck catching corner of rib. Lump of coal 3"x9" come into oper. deck. Oper tried to move with left foot while s/car still tramming, causing a laceration to inside of ankle.

Rock fall in the face area of the no. 5 entry which entrapped the Gaylis 300 roof bolter approx 80' in by spad 4958 on the 001 unit. The machine will not be recovered. Fall was approx 5' high 40' long 20' wide.

Didn't fall on working section. Discovered on a development section 1st Right Mains during removal of mining equipment from area due to encountering adverse roof conditions in both the outby areas & immediate working face. Fall was discovered after decision to pull off this section area. Fall was 30'length-20'wide-5'high. 1st right mains between 4+5 right of spad 4917.

THIS WAS A ROCK FALL LOCATED 3 BRKS ABOVE SPAD 1987 + OVER IN THE #3 ENTRY IN THE INTERSECTION. THIS AREA WAS BOLTED WITH FULLY GROUTED RESIN BOLTS, PARTLY CRIBBED + TIMBERED. THIS AREA WHICH IS OUTBY IMMEDIATE WORK AREA 15,000' HAS HAD SOME ADVERSE CONDITIONS. 35'LONG, 20'WIDE, 10'THICK.

EE WAS RIDING A GOLFCART DOWN THE #4 BELTLINE WHEN HE DROVE OUT OF THE ROAD STRIKING A TIMBER, KNOCKING A ROCK DOWN ON HIM. THE ROCK CRUMBLED WHEN IT HIT HIM, IT WAS 18" X 5" X 14". IT BRUISED HIS LEFT LEG MUSCLE & HIS LEFT RIBS WERE ALSO BRUISED.

THIS WAS A ROCK FALL 1 BRK ABOVE SPAD 4571 WHICH WAS SPAD 4572 ON THE BELT LINE OF THE NO. 5 BELT ON THE .001-002 UNIT. IN THE NO. 4 ENTRY. THE FALL WAS 20' LONG 60' WIDE 8 OR 9' HIGH. THIS FALL WAS 1000' OUTBY THE WORK AREA. THIS WAS 3-22-04 BETWEEN 6 OR 6:30 AM.

EE WAS BOLTING 6 RT BREAK OUT BY 6 HEADING OF THE 003 UNIT. A PIECE OF DRAW ROCK FELL OUT ON THE LEFT RIB 3' THICK 3' WIDE AND 5' LONG. IT FELL ON BOOM OF ROOF BOLTER, IT BROKE AND HIT EE ON LEFT SHOULDER BRUISING IT.

EMPLOYEE WAS HELPING FIX A WATERLINE WHEN THE BAND IN THE TOOL BROKE AND SLIPPED CAUSING HIM TO CUT HIS LEFT WRIST. IT TOOK 15 STITCHES TO FIX HIS ARM.

EE HAD AN ALUMINUM LADDER ON THE SIDE OF A TRUCKBED, DEICEING IT. THE LADDER HAD ICE UNDER IT. THE LADDER SLIPPED CAUSING HIM TO FALL, HE FELL ON HIS LT. SIDE. EE BROKE HIS LT. COLLAR BONE & HAS A CONCUSSION ON THE LEFT SIDE OF HIS BRAIN.
