As injured employee was walking EE wasn't watching EE was going and EE's knee bumped the sledge hammer handle that EE had previously placed on the floor adjacent to the track mobile. The injured employee grabbed for the hammer handle to prevent it from falling to the floor and the same time the right ankle turned inward.

EE was working outside using a 20lb sledge hammer to loosen material that was stuck inside the chute. EE went in to cool off. About 10 minutes after EE tried calling to get ahold of someone because EE stated was cramping up and skin was hot clammy and EE was shivering.

A Caterpillar D7 was used to push stripping material over the edge of the spoil pile during rainy, wet conditions and at night. The dozer operator stated that ee got too close to the edge and come to rest sideways, approximately 100' below. On April 7 it became a medical incident due to the employee receiving physical therapy.

Employee was working at the Secondary Plant on Station #2. They were sliding screens out. He was trying to lift himself up on to the end of the 2nd deck so he could help get the screens out. When putting all his weight on his arms, his right shoulder gave away and popped out of place.

Scale clerk was operating the 980G loader. While navigating a decline, the loader began to pick up speed. The operator panicked & could not locate brake pedal. While attempting to steer thru a curve, the operator steered too far to the right which deflected the loader to the left. Left front tire hit a soft low spot overturning loader into a settlement pond.

Employee was removing bolts from the conveyor legs. As the bolts came loose, the conveyor leg swung out and hit the ladder he was standing on and he fell.
Moving an electric motor up stairs, it started to slip, as he went to hold if from falling, strained his back. Did not seek medical attention until 7/26/07.
