Employee slipped and fell while descending a sloped gravel embankment after completing an inspection at the screen tower. The employee sustained a fractured left fibula.

While using a box cutter knife employee cut left leg, which resulted in 8 stitches.

Employee was picking up one side of a piece of metal. When employee lifted one side of the metal the bottom piece of metal slid forward and landed on the employee's left foot.

Employee was checking the screen on top of the crushing plant. As he started back down the ladder, he did not have his eyes on path and his foot missed the top step of the ladder causing him to fall approximately 8 feet to the catwalk located on the next level below.

EE WAS MOVING A BOX OF CORES. HE FELT A PAIN IN HIS LOWER BACK.
