Employee was climbing down off a D11 bulldozer and slipped on the muddy track, when EE slipped EE pulled something in the shoulder. EE was released to full duty that day but after a CScan in June they found a slight tear in EE's shoulder.

Showing all 12According to driver, got loaded in the pit and started pulling ramp up out of pit got about halfway up the ramp and broke traction and started sliding backwards and went sideways and upset. It had been raining off and on all morning
Operator running a machine noticed that there was oil coming out of a Trac-Hoe running in the area and before being able to warn the operator the machine caught fire. The fire burnt the wire to the shut off so the pumps kept running and feeding the fire. Operators walked away and called for the fire department. Operators done as they were trained to do. No one hurt!
EE was shoveling around mesh belt guard of crusher. EE hit his right hand on the mesh guard, which caused a laceration to his hand. Approx. 10 stiches were needed to close wound, he was able to RTW immediately.
EE was moving backhoe down a ramp into the coal pit. The door on the machine was in the open position, the operator thought the door was latched in the open position. The door was not latched so while he was moving down the ramp he was holding onto the door jamb of the machine. The machine hit a bump causing the door to swing closed on his hand.
Employee was attempting to hook boom truck to shaker screen. He was holding hook to get aligned with screen (the hook is on a swivel) & when they picked up the screen, the swivel turned & pinched his right pinky finger against the shaker screen. His finger was lacerated on top & bottom. Stitches were needed to close the wound.
The auger was found in the early morning covered with rock. The auger had been left up against the highwall because there were two holes left to be augered.
Rain had occurred a couple of days prior to accident; also heavy rain was present at time of accident. While contractor was loading blasting holes next to highwall, a rock fell from the highwall, hitting the man in his left leg. Man was taken to hospital. He was diagnosed with a left lower shin bruise.
Employee was trying to see if the D-11 dozer he was operating had an antifreeze leak. he climbed down from the track of the dozer (the track was muddy) he slipped and fell on his right forearm.
EMPLOYEE WAS CLEANING A MOTOR ON A COAL CRUSHER WHEN A PIECE OF METAL (RUST) FLEW INTO HIS LEFT EYE.
EMPLOYEE WAS STANDING ON A ROCK WHILE HE WAS CONNECTING WATER HOSES TOGETHER (FOR A WATER PUMP). HE SLIPPED OFF OF THE ROCK AND TWISTED HIS RIGHT KNEE.
EE WAS PICKING UP A SUD SAVER DRILL BIT COUPLING 80 LBX APPROX. TO PLACE IT ON DRILL. AS HE WAS LIFTING SUB SAVER UP, WHEN HE GOT ABOUT WAIST HIGH APPROX. 3 FT OFF OF GROUND HE FELT SOMETHING POP IN HIS MID BACK.
