Employee was roof bolting drilling an hole and had left hand on pod when a rock measuring 30 inches long, 6 inches wide and 5.5 inches thick fell out striking their left hand. Employee was treated for fractured ring finger and lacerated pinky.

An unintentional roof fall happened on the S3C-2nd Right entry 7 crosscut 7. It fell out in an intersection on top of a continuous miner. It is believed that the water above the stack rock deteriorated the rock strata causing it to fall. A site specific plan was submitted and approved to clean rock off continuous miner to retrieve it from the fall.

On 02/15/2026 an unintentional roof fall occurred On the P3-1 (Unit #3) at crosscut #18, entry #3 in the last open crosscut. The fall is approximately 20 foot in length, 30 foot wide and 8 foot thick. It seemed the water bearing sandstone interval came down to approximately 8 foot, causing the immediate roof to fail.

on 1/29/2026 an unintentional roof fall occurred on the #35 cross cut entry #5(belt line)of the 2C. The fall is approximately 20 foot in length, 28 foot wide, and 15 foot thick. It is believed that water deteriorated the upper strata making it unstable, causing it to fall.

Employee was working on a battery coal hauler in diesel shop when they stepped up on the coal hauler tail-gate with right leg and felt left knee pop. On 2/6/2026 we were advised they had a meniscal tear and was placed on restrictions (No Kneeling).

Employee was taking down fire suppression above a scoop charger. When employee stepped off charger EE stepped on rock, rolling EE's ankle. Ankle was found to be sprained and the employee was put on restrictions (mainly sit down work only) and placed in a walking boot.

Employee was trying to break apart a threaded water-line coupling, straining left shoulder. Employee has had MRI. On 10/29/2025 MRI results showed 2 tears in shoulder. Was seen by Ortho on 10/30/2025 and has surgery pending. Employee remained working till 10/29/2025.

Employee was pulling on belt jumper at Unit 2 beltline cross cut #1, entry #5, when they felt a burning pain in pelvic area. On 12/08/2025 employee was diagnosed with a hernia. Employee has worked normal duties and will continue to work until surgery date 12/30/2025.

Employee states they were lifting an equipment door when they heard and felt their right arm popping in the bicep and elbow area. MRI has shown a torn bicep muscle and the employee is scheduled for surgery. Employee has continued to perform normal work duties.

Employee was operating miner, when a rock measuring approximately 2ft.x2ft.x1inch fell out between hog panel and coal rib. Striking employee on hard hat and shoulder area. Employee has continued to work until 10/29/2025 when they were placed on restrictions, pending physical therapy.

Injured was pulling on top roller on 4/8/25 and felt pain in arm but continued to work. On 5/13/25 injured was seen by doctor as EE's pain had not went away at that time injured was diagnosed with a partial bicep tear and placed on restrictions. With a possible surgery for repair scheduled.
