Showing all 5AT APPROX. 8:30PM #3 CAR BREAKER KNOCKED. CAR DRIVER WENT TO RESET BREAKER & INJURED EE WENT TO SEE WHAT WAS WRONG WITH CAR. HE HAD ONE KNEE ON THE GROUND & LEFT HAND ON CAR. WHEN BREAKER WAS PUT BACK IN HE RECEIVED A SHOCK. BREAKER DID NOT STAY IN. INJURED EE WANTED TO WORK REST OF SHIFT BUT WAS TAKEN TO HOSPITAL TO BE EXAMINED.
EMPLOYEE WAS PUSHING BOLT IN HOLE, BOLT SLID AND PLATE STRUCK EMPLOYEE ON RT. HAND- EMPLOYEE WAS TAKEN TO E.R. FOR EXAM AND POST ACCIDENT DRUG TEST.
Employee had bent a roof bolt. While using the head of the bolter to push bolt in hole, the bolt slipped off and hit him on the right side of his face. Employee was sent to the ER for treatment. He received one stitch to the right side of his face and returned to work.
EE STEPPED ON A NAIL THAT WAS DROVE THROUGH A BIT BUCKET LID. PUNCTURE WOUND TO LEFT FOOT. EE WENT TO DR AFTER HE LEFT WORK. OFF WORK 4 DAYS. RETURN 4-16-04 FULL DUTY.
