Struck by falling object

Operator was drilling a hole with a TD230 bolting machine. A piece of drawrock fell onto the ATRS, when he lowered ATRS, the rock slid off the machine, scratching his back. Also, the drill steel fell against his neck, buring his neck.

Employee twisted around while underground pulling muscle in his back & neck.

Accident type, without injuries

A roof fall measuring 40' long - 15' wide and 6' high fell between blocks at break 42. The roof fall consisted of lamenated shell. The area was timbered and cribbed and reported to MSHA.

Employee slipped his foot went under head of bolting machine, as he tried to break his fall he grabbed the lever that lowered the head onto his foot.

EMPLOYEE SAID HE HURT HIS BACK WHILE PACKING A BAG OF ROCK DUST. ON 9/28/04 EE WENT TO SEE A DR. ABOUT HIS BACK. EE IS OFF WORK UNTIL AN MRI IS PERFORMED. PENDING COMP. CASE.

AT APPROX. 8:30PM #3 CAR BREAKER KNOCKED. CAR DRIVER WENT TO RESET BREAKER & INJURED EE WENT TO SEE WHAT WAS WRONG WITH CAR. HE HAD ONE KNEE ON THE GROUND & LEFT HAND ON CAR. WHEN BREAKER WAS PUT BACK IN HE RECEIVED A SHOCK. BREAKER DID NOT STAY IN. INJURED EE WANTED TO WORK REST OF SHIFT BUT WAS TAKEN TO HOSPITAL TO BE EXAMINED.

EMPLOYEE WAS PUSHING BOLT IN HOLE, BOLT SLID AND PLATE STRUCK EMPLOYEE ON RT. HAND- EMPLOYEE WAS TAKEN TO E.R. FOR EXAM AND POST ACCIDENT DRUG TEST.

Struck by falling object

Employee had bent a roof bolt. While using the head of the bolter to push bolt in hole, the bolt slipped off and hit him on the right side of his face. Employee was sent to the ER for treatment. He received one stitch to the right side of his face and returned to work.

EE STEPPED ON A NAIL THAT WAS DROVE THROUGH A BIT BUCKET LID. PUNCTURE WOUND TO LEFT FOOT. EE WENT TO DR AFTER HE LEFT WORK. OFF WORK 4 DAYS. RETURN 4-16-04 FULL DUTY.

A SHUTTLE CAR CABLE CAUGHT HIS LEG AGAINST THE COAL RIB WHILE TRAMMING THE CAR TO DUMP COAL. SPRAINED RIGHT KNEE.

WHILE RIDING IN A SCOOP BUCKET AS A MANTRIP, THE SCOOP WENT THRU A DIP AND WHEN THE SCOOP CAME UP THE BUCKET WAS RAISED, RUBBING THE EMPLOYEE INTO THE MINE ROCK (BRUISED RIBS).

THE ABOVE MINE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL IN #5 ENTRY IN AN INTERSECTION 2 X-CUTS INBY SPAD NO.152. THE FALL WAS APPROX. 5-6'THICK, 25'WIDE, AND 40'LONG WITH LAMINATED SLATE AND SOME SANDSTONE.

EMPLOYEE WAS TRYING TO REMOVE A PIECE OF DRILL STEEL AND CAUGHT HAND BETWEEN DRILL POT AND MINE ROOF CAUSING A FRACTURE AND LACERATION TO RIGHT THUMB AND HAND.
