While getting off the water truck, the employee stated that when he got to the last step his knee gave way causing him to slip and fall.
Changing hoist cylinder on 793B truck, half header fell out and struck employee on lower leg
Employee was using a swivel socket to remove bolt. The pin came out of the socket and struck employees left index finger and thumb.
Employee was on rear of grader fixing a light when he turned around to get in the cab stepping on tire. He slipped and fell between the tire and frame of machine striking his face and knee on final drive of the 16th grader.
EE was removing exhaust pipe from 793B truck, lost footing and slipped causing ee to strike his elbow on step/platform on the engine.
EE was helping to move the forks on a fork loader his hand was caught between the forks and the back board of the loader.
At the end of shift employee was stepping off of D-11-R dozer. When stepping to ground, he turned his left ankle on the uneven ground.
Was throwing end cap off of machine down to mechanics in the group so they could put back on the shovel. EE threw the end cap down, there was a sharp edge on the cap that caught his ringer finger and cut the end of his finger. EMT cleaned and bandaged.
EE was cutting bolts out of cutter head motor #2 side of No 1 highwall miner. When the act hose burst, the torch ignited the gas where the EE was cutting.
Stepped out of ladder and twisted his ankle.
Walking off push beam in table, put his left hand on table for balance. When he stepped off push beam a piece of metal broke off in employee's left palm.
Working on hose red chain. Trying to take apart, wrench slipped, causing hand to be cut on palm of hand (L) side.
Grinding cutter drum preparing to weld bit lugs to drum. Metal got into eyes. Employee did have safety glasses on at time of accident.
EE was putting the belly pan back on the miner when the lifting eye broke off letting the belly pan drop approx. 16" landing on the employees legs.
On oct 3rd, EE reported that on 9/20/07, while pulling on a cable he injured his back.
Employee was filling a hydraulic tank with oil when he slipped and fell.
Employee fell between the miner power head and a push beam "car." The push beam was knocked forward causing employee's leg to be caught between powerhead and push beam.

EE ALLEGES HE WAS MOVING SPOIL MATERIAL WITH LOADER BUCKET. THE MACHINE TRAMMED OVER UNEVEN PIT FLOOR. THE REAR TIRES BOUNCED OVER 36" HOLE IN PIT FLOOR. (SPRAINED BACK FROM SUDDEN JOLT & BOUNCE OF LOADER.)
Employee was striking a link pin with a ball peen hammer. Small piece of metal struck and embedded in left wrist.
(LOCATION: ON HAUL ROAD AT PUNCHEON CAMP THIN SEAM MINE - MSHA ID NO. 4609077.) EMPLOYEE WAS WALKING BESIDE THE HIGHWALL MINER AS IT WAS BEING TRAMMED. A PIECE OF METAL (3/4") BROKE FROM CRAWLER TRACK AND SHOT OUT, STRIKING EMPLOYEE IN LEFT ARM.
Employee was welding.
Employee was removing a grease fitting on the crawler unit. the fitting blew out and struck ee s hand.
EMPLOYEE WAS HELPING TO "TEAR DOWN" THE MINER, WHILE HOLDING A PIN, THE PIN SUDDENLY CAME OUT & A LATCH FELL STRIKING EE ON HIS HAND. "REMOVING CUTTER HEAD"
