Employee was descending from D11R dozer (Company No. 394) when employee slipped and fell 3 feet onto the fender striking his right hip against the machine.

Employee was getting off dozer when a piece of clothing got caught causing him to fall back toward the dozer. Employee struck his head and shoulder on the dozer.

While having truck loaded, a rock came in contract with the truck and jarred the employee.

Employee was getting off the loader, he was on the bottom step and lost his balance causing him to slip and fall.

Over-exertion in lifting objects

Employee bent down to pick up a box of caps. He stated he felt a sharp pain in his back and fell to his knees. After he set down for a few minutes he was able to return to work with no problems. The next day he felt pain in his back again and went to the doctor.

EE was pushing in a shot. The dozer slipped and then caught traction. This sudden jerk caused EE's neck to be jerked.

EE stated that while getting off a D-10 Dozer, he slipped and pulled a muscle in his back.

Employee was operating a rock truck and while being loaded, a large rock fell out of the loader bucket into the truck bed causing him to be bounced. Employee did not miss work until 10/27/11.

Employee had installed engine and was exiting the pump room. He slipped on a wet walkway and caught his hand on sheet metal in the doorway, causing laceration to thumb.

Installing steering clutch in D10R. While wrapping the track, the link engaged in sprocket and a piece of metal struck ee in right forearm causing a puncture. Metal sliver was embedded in arm.

EE stated that while getting up on the steps of the rock truck, his ankle gave out and twisted.

Employee stated that while getting on the lozer, he had 2 hands on the handrail and 1 foot on the ladder, while pulling himself up he heard a pop in his right arm.

Employee was injured on 4-11-11. While operating a rock truck he backed over a rock and jarred his back. Employee returned to work the next day and was able to work until 4-18-11. Employee stated that pain was getting worse and he was unable to work.

EE stated that while operating a rock truck, he ran over a rough spot in the road and caused him to be jarred in the seat. EE stated that this caused him to have lower back pain.

EE walked to right side above engine compartment when he slipped on the deck of the rock truck catching himself with his other foot resulting in twisting his back.

Employee was on rear of grader fixing a light when he turned around to get in the cab stepping on tire. He slipped and fell between the tire and frame of machine striking his face and knee on final drive of the 16th grader.

At the end of shift employee was stepping off of D-11-R dozer. When stepping to ground, he turned his left ankle on the uneven ground.

Was throwing end cap off of machine down to mechanics in the group so they could put back on the shovel. EE threw the end cap down, there was a sharp edge on the cap that caught his ringer finger and cut the end of his finger. EMT cleaned and bandaged.

Employee had entered cab of haul truck and was about to close the door when a gust of wind blew the door shut and caught the employees little finger against the door jamb.

Employee was driving fuel truck, traveling to Johnson Hollow Pit. Employee stated that the inside tire of the rear right axle blew out. He stopped the truck to look at rear wheel tires & discovered a rock wedged between the tires. He then reached over into the wheel well to remove the rock when the outside tire blew out striking his right hand.

Employee was loading crib blocks into the school bus ""man trip"" when one of the crib blocks slid down the bus steps and struck employee on the knee.

Employee was exiting the cab when he slipped on the floor causing him to fall forward and hit his head and left shoulder on the gear box frame.

Employee notified management on 1/29/08 that on 1/26/08 while operating a rock truck, the loader bucket dropped and hit the truck bed. Employee stated that this hurt his head, neck, and back.

Employee stated that on 12-12-07 while operating the end loader, the bucket on the loader slipped off the corner of a rock and this caused him to be jarred. Employee did not miss work and did not go to the doctor until 2-9-08.

As employee was dismounting a D-11 dozer he stepped on a rock that was lodged in the step and fell backwards to the ground. He struck his head on a rock causing a laceration above his left ear.

The 785 Cat. rock truck was being loaded by the 5230 Cato shovel. The bucket of the shovel struck the bed of the rock truck. The jolt from the bucket striking the bed injured the truck operator's back.

The dozer trammed too close to the edge of the spoil bank. The spoil bank failed and the dozer dropped over a steep embankment about 30 ft high.

Employee was installing a track pin and while driving the pin in with a sledge hammer, part of the pin flew off striking employee in the left cheek; metal went through cheek into his left eye, damaging the eye lens and detaching the retina. Employee went to Bowman Gray Medical Center for surgery where the lens was removed;

The APC Truck began to go up a hill, the brakes gave way and the truck began rolling back into an embankment, tipping the truck over. Before the truck tipped over John jumped out hurting his left foot.

Employee was under sitting front end loader remove the transmission drain plug. the wrench slipped out of the socket and he fell backwards ee's elbow struck a 4"" rock.

EE WAS OPERATING A 777 CATERPILLAR ROCK TRUCK AND WHILE BEING LOADED, A PIECE OF ROCK FROM THE LOADER BUCKET FELL ON THE TRUCK BED. THIS BOUNCED THE EE. EE STATED THAT HE FELT A PAIN IN HIS NECK AND ARM. AS OF 5/24/04, EE IS WAITING ON RESULTS OF MRI EXAM.

MECHANIC HAD REMOVED AND DISSAMBELED A 785 TRUCK FRONT BRAKE PACK. AFTER REPAIRS WERE MADE & DURING THE REASSEMBLY PROCESS, A HOIST CHAIN SLIPPED ALLOWING THE TOP SECTION OF THE BRAKE PACK TO DROP & SMASH EE'S FINGER BETWEEN TOP HALF AND MIDDLE SECTION.
