While operating a 70-ton rigid frame haul truck, EE backed up to the berm on the South end of the pad. It appears the stockpile under the back wheels fell away from under the truck, causing the truck to slide off the stockpile and turned over backwards. It appears that EE impacted the roof of the cab, causing serious injury.
Picking up a wooden crate standing on top of a small dirt mound, lost balance and right knee buckled
Helping plant mechanic remove teeth from caterpillar excavator. Piece of sharp metal broke off striking EE in the left arm
Loosening bolts on the 2nd deck of the wash plant using a crescent wrench when wrench slipped causing left hand to slam into sharp metal corner puncturing EE's glove, leaving a deep puncture wound in left hand.
