Observing while standing clear of 6 inch Driscopipe being removed. Driscopipe slipped hitting the rail of the conveyor belt pushing Driscopipe outwards striking employee blocking with both hands.

REPLACING CUTTING EDGES ON LOADER THE OPERATOR WAS USING LEFT INDEX FINGER TO HOLD A WORN CARRIAGE BOLT IN PLACE SO THAT THEY COULD LOOSEN IT. WHEN THEY HIT IT WITH IMPACT WRENCH IT SPUN CUTTING THE FINGER WITH THE SHARP EDGE

Loosening bolts on the 2nd deck of the wash plant using a crescent wrench when wrench slipped causing left hand to slam into sharp metal corner puncturing EE's glove, leaving a deep puncture wound in left hand.
