Showing all 8He was helping replace a packer. He bent down to pick something up and hit his neck on a super sack fill pipe when he stood up.
Injured EE and another EE were trying to remove a bearing with a puller and a hydraulic pump, when the puller slipped and hit EE in the head and shoulder. EE received 4 stitches, he did have his hard har and safety glasses on.
Working on a screening unit, did not have it rigged proper, lid broke and fell about 4 inches. Did not slide unit out of way. Had arms under unit.
Was helping maintenance change screen. Maintenance had unit in air, not rigged proper and not slid out of way. Lid broke and fell 4 inchs and hit his arm.
