Cutting left hand equalizer bar end pin out of roller frame and slag popped off went into right ear causing a thermal burn.
The articulating truck blew a steering hose near the turbo causing a fire. The operator attempted to extinguish the fire but had to call the fire department. The rock truck cab was a total loss but no one was injured.
EE left work on 02/02/2016 without reporting an accident. EE reported to the hospital the following afternoon and was treated and released for a back sprain. Dr excuse said to return to work the following day 02/04/2016. EE has not yet returned to work. EE did call on 02/15/2016 and said EE would return next day. Has not returned to work as of 02/21/2016.
Torqueing mount bolts for 992G wheel loader unit and ratchet gear stripped causing employee to fall backwards straining lower back

Employee alleges injury after being laid off at the end of his 12/5/2013 shift but never reported to management. Management was notified on 12/20/2013 by Workers Comp. Claims neck and back injury after running his rock truck into another rock truck.
EE was dismounting grader slipped and fell to ground.

While installing engine exhaust manifold and engine turbo. The technician had to work in a bent over posture for an extended period of time. No pain was felt on the day he was performing the work. The technician began noticing soreness and stiffness in his right lower back on 8/26.
Fixing pump and was climbing out of pit. Co-EE reached to give him a helping hand up and IEE slipped as co-EE was pulling him up. Heard something pop in his back. Right leg went numb, left leg and lower back had a lot of pain.
Employee alleges falling from a 777 rock truck causing back and arm injury.
The operator was dismounting the DM Ingersol Drill when he fell. Sliding ladder was maintained in up position three point contact was not being utilized. No oil/grease was found on hand rails, ladder nor were any stumbling hazards found. The operator complained of lower back and hip pain and was taken to the hospital. No witnesses were present at the time of fall.
Employee became light headed while operating his 785 rock truck and thought he had a seizure. Dr. diagnosed as heat stroke.
Employee was walking to his service truck and tripped spraining left ankle.
Step broke on drill when operator was exiting drill. He slipped and fell hitting his head.

Moving launch on highwall miner getting ready to pull out when hose burst and struck victim in the back of the head and at the base of the skull. Temporarily knocked him out while knocking him onto the launch pad.
Pushing on a shot and backed over a big rock. Jarred the victim. Had pain running from right hip to right ankle. Was x-rayed and released.
Victim was twisting the belly pan on a dozer to put on right way and chain slipped and unhooked and caught finger against the track. Pinched and cut left pointing finger. X-rayed with no broken bones and had to have stitches.
Stepped down in a low place and slipped and fell. Foreman asked him if he was hurt. Victim stated he was fine and did not want to seek medical help. On March 23rd, victim showed up to work in the parking area and became very nervous acting. His son who is also an employee took him to the doctor.
Step broke on his drill when he got off to mark holes. Shinned right leg on his shin. Required stitches.
Pushing shot to loader and backed over a big rock. Dozer went backward over rock then forward throwing employee forward. Pulled muscle in stomach area.
Helping mechanics work on steering on RT#099. Mechanic was in truck and employee was at left side. As mechanic went to check steering, victim went behind left front tire and was pushed into housing. Fractured facial bones on left side and had bloody nose.
Dismounting D10N dozer and foot slipped. Step was wet and foot slipped off the second step causing pain in the lower back going out through the right arm.
Operator was backing up the dozer when he hit a rock with one track. Said his neck was stiff and went to hospital the next day. Said he had a pulled muscle.
Picking up rock off prepping car for highwall miner and co-worker dropped pin in tray when it bounced and hit right thumb and hand braking the right thumb just about the joint.
Welding on bucket of excavator and the surface area was slick. Safety glasses fell off face and when he reached for them he slipped and twisted knee.
Wrench slipped while tightening poly-belt that runs between motor and gear reducer on incline belt of highwall miner. Resulting in a laceration over his left eye that required stitches to close.
Employee was going to show co-worker how to hook up a chain. Walked by conveyor/MBC unit and cable arced/blowed out. Stuck left hand out to block flash and ran away from arcing. Received burns to hand and blistering to left side of face.
Hauling load in 777C and went from wet to dry conditions while coming down embankment. Appears tires caught while getting ready to dump and truck tipped over onto passenger side. EE was sent to hospital with abrasions and released 11/09/2011.
EE was climbing off the dozer and his feet slipped. He caught himself on the push arm and pulled a muscle in his back. Taken to emergency room by his wife. Given cat scan, given muscle relaxers and missed one shift of work. Returned to work with no restriction.
The victim was operating a rock truck. When the shovel was loading the truck, a rock shook the truck and jarred the operator. He began experiencing pain in his neck area. Diagnosed as bruised cervical. Returned to work on 10/17/11 with no restrictions.
EE was climbing onto the rock truck at beginning of shift when his back popped. Later while being loaded his truck was shook by a large rock and his back popped again. Taken to emergency room, cat scan, given pain medication and off work for 2 days. Returned to work with no restrictions on 10-17-11.
The victim was operating a rock truck. When the shovel was loading the truck, a rock shook the truck and jarred the operator. He began experiencing pain in his lower back and groin area.
