Fixing pump and was climbing out of pit. Co-EE reached to give him a helping hand up and IEE slipped as co-EE was pulling him up. Heard something pop in his back. Right leg went numb, left leg and lower back had a lot of pain.

Struck against a moving object

Pushing shot to loader and backed over a big rock. Dozer went backward over rock then forward throwing employee forward. Pulled muscle in stomach area.

Struck against stationary object

Helping mechanics work on steering on RT#099. Mechanic was in truck and employee was at left side. As mechanic went to check steering, victim went behind left front tire and was pushed into housing. Fractured facial bones on left side and had bloody nose.

Fall from machine

Victim was stepping onto the bottom rung of the ladder to climb onto the Caterpillar 777 he was operating when his foot slipped. He sustained an avulsion fracture to his right ankle. He took himself to the hospital. They x-rayed, and splinted his ankle, he was released and returned to work on the 14th with no restrictions. Missed 3 days work.

Caught in, under or between a moving and a stationary object

Victim was installing a top track roller on a D-9 dozer. The roller slipped and caught his middle finger on right hand between roller and frame. Fractured middle finger. Employee took himself to the hospital the next day. They x-rayed, cleaned and splinted the finger and he returned to work that day.

Struck against a moving object

Victim was operating a 992D loader in shot material, a rock fell off the highwall and struck the loader bucket boom, causing him to be shaken in the seat. He started having pain and burning in his neck . He worked until the 21st when he went to the doctor. They did a catscan, gave him pain medicine and a pain shot. He returned to work & no time was lost.

Fall onto or against objects

Victim was climbing down the stairs and slipped on the bottom step. He fell and struck his buttocks on the steps. He bruised his lower back and buttocks. Employee was off the 15th and returned to work on the 16th & 17th . He worked one-half day on the 18th and said his back started hurting again so he went back to the Pikeville Medical Center.

Employee was hauling tires onto job site. While removing binders holding the load, the binder came loose and struck him in the eye.

EE states that back was injured at another location in 4/08.While driving the ANFO truck on 4-21 he hit a small pothole & pain went up his back & neck.On 4/25 treatment was started for muscle spasms,on 5-1 notified supervisor that he would be off work & had been put on steroids.Still off work to date.

Removing and installing segment on D11 Dozer. EE did not miss any work with this injury at first. His back was still bothering him so he went to Dr. on 4/20/06. & Dr. took him off work. EE is currently off work due to injury on 3/10/06.

REMOVING PIVOT SHAFT RETAINER PLATE FROM DOZER BY APPLYING AIR PRESSURE. WHEN RETAINER PLATE EJECTED FROM ROLLER FRAME HOUSING, IT STRUCK EE IN THE STOMACH AND RT. SIDE.

Struck against stationary object

Victim struck his head on a cross member of the Anfo Bin while preparing to load a truck with Anfo. Six staples to close the wound. Employee was a summer employee and scheduled for 08/20/2004 to be his last day of work before returning to school. He just asked to be terminated one day prior to schedule. No lost time involved and voluntary termination.

Victim was working on a dozer when he slipped & hurt his back. He is being treated for a herniated disk in his back. He continued to work through June 21, 2004 and began missing work on June 22, 2004 and is still off work.

Fall from machine

Operator climbed into dozer to start work shift, got back out to clean the windows and fell backwards onto the tracks and then to the ground. Bruising to this left hip and shoulder. Small laceration on left hand. Small laceration on left hip which required one stitch to close.

Operator backed dozer over a rock while pushing in shot material. caused him to be jerked quickly. Complains of pain in lower back.

Victim was operating a D-11-N dozer approx 15 ft from the highwall. A rock approx 12"" x 4"" broke loose from the highwall & went through the left door window striking him in the lower leg. The highwall had been scaled, however a lot of rain had fell in the previous couple of days & probably contributed to the rock coming out of the higwall. Compound fracture to lower left leg.

OPERATOR FAILED TO MAKE SURE STEP WAS FREE OF DEBRIS BEFORE CLIMBING DOWN. WHICH RESULTED IN HIS FALLING ON HIS BACK.

EE WAS MOUNTING A TIRE AND ROLLING A WHEEL PART BEAD SEAT BAND ONT HE GROUND TO PUT ON TIRE AND IT HAD A BURR STICKING OUT HE DID NOT SEE AND CUT HIS LEFT INDEX FINGER.
