Showing 50 of 54EE was dumping a load of material out of the box of a haul truck. The cab was turned slightly which caused the cab to tip slowly on the left side causing EE to slam left hip into armrest in cab
EE was in haul truck being loaded. The excavator scooped the 4th bucket, swung around to dump in truck but didn't have bucket high enough and hit left hand corner of haul truck box.
While pulling on a wrench to loosen a rusty bolt on a conveyor, EE felt a sharp pain in their back and front groin.
While using an angle grinder to assist with welding, a piece of metal went under EE's safety glasses.
Fixing a bearing on a sand screw. Stepping on a ladder to come down and the ladder fell, EE landing on the ground
EE was getting into a loader and as EE was pulling self up EE felt severe pain in right lower back.
EE was emptying a stone hopper by hand and felt a strain in left shoulder after they were done. Reported 8-27-2020 did not see doctor until 9-30-2020.
Lifting piece of metal kneeling on one knee, removing bolts to get rubber off of metal piece.

EE was removing a cutting edge from a loader bucket with an air impact when the bolt spun, grabbed EE's glove and ripped through it, cutting EE's left hand palm and wrist.
The hydraulic ram broke on the dump & came down quickly. Shook up the driver inside the cab.
Employee was trying to dislodge a rock from the back of truck as the rock fell, ee fell backwards fracturing the 7th rib on left side and hurting left elbow.
EE Pulling on a wrench to tighten a bolt when the wrench slipped off the bolt and EE bumped their back on a rail as they moved backwards with the momentum. **Employee did not report this injury until 10-20-17**
Unchaining the pin on a sand screw. Stepped back awkwardly and twisted back.
Employee was getting off the seat of the end-loader when employee felt pain in lower right back and right leg.
Employee was getting out of loader after EE had finished loading the dump truck, when the last step on the loader bent inward towards the loader causing employee to fall backwards and to strain right knee.

Employee was in the excavator digging in the pit when the bucket struck a ground nest of bees and one flew up and stung employee in the face.
EE was removing a guard off conveyor C15 for routine maintenance. Using force to remove the guard by hand and right thumb struck a piece of metal. Skin was removed off the top of the right thumb knuckle area.
EE was repairing the UFR pump when the pump cover closed without warning and pinched EE's left hand ring finger.
EE was inspecting the equipment on an excavator, lifted the hood to check the oil level when the hood came down on shoulders and head.
Working on a conveyor, went to lift the guard when he felt pain in his lower back. Didn't treat until 11/16/2015.

Employee strained his lower back on 5/29/15 while changing a hydraulic hose on a drill. He did not seek treatment until 6/2/15. He has been taken off of work.
Installing a new conveyor belt on a concrete sand stacker. While unrolling the belt that was on a metal shaft, the employee placed the belt on the forks of a forklift. The roll twisted off the forks striking him in the right leg causing contusion and bruising to his right leg inside the knee area.
Lifting a screen in place on the top deck of screener
Right ring finger was crushed between timber and blocks causing an abscess of the nail and eventually the nail to fall off.
Stepped on a rock after stepping down from a ladder and rolled his left ankle. Causing a sprain/strain to his left ankle.
Employee was operating the Sure-Strike machine breaking boulders into smaller pieces when a busted piece came through the right side window striking employee in the mouth. The piece of rock chipped his right front tooth, and he experienced pain in his lips.
Employee was picking up a large rock off of the table at the crushing plant. When he felt a "pop" in his left arm. Employee was diagnosed with left wrist strain and shoulder pain.
Repetitive use of break pedal in pay loader caused left foot pain and swelling. Left foot injury.
To our knowledge, employee was getting out of a piece of equipment when he twisted his lower back and felt pain.
EE was pushing oversized fieldstone over the side of the belt, when the stone he was pushing rolled and struck another getting ee finger smashed. His left middle finger was smashed; Diagnosis was a tuft fracture to his left middle finger. Finger was splinted.
Employee was cutting a rubber mat with a utility knife when the utility knife slipped off of the rail cutting the employee s lower right leg.
EE was picking up 8" - 12" fieldstone from off of the crusher pitching table. As he picked up a 12" rock to throw it into the truck, he felt pain in his right forearm. Diagnosis was right forearm tendonitis.
EE was replacing teeth on the front end loader, when the hammer head flew off the handle, causing a contusin of his left tibia.
EE was unloading a PTO shaft for cone @ Metal Crafters for repair. EE of Metal Crafters was helping that person, dropped his end without warning EE. EE was still holding on and caused a shock to hip and lower back. This resulted in a sharp stabbing pain in his lower back and continured discomfort.
EE was working on the suction hose of the water pump. He was checking the bolts that were previously tightened. EE was tightening 6" suction hose on water pump and felt a strain in his neck.
While grinding a welding edge, a piece of metal went under his safety glasses and embedded into his right eye.
While working twisted his right ankle.
Employee was serving a 5'x5' steel mesh screen. He was bent over at the waist and tried to move the screen by pushing it across the floor, and strained his lower back int he process.
Walking out door, dust or shaving blew into left outside edge of eye. Employee was treated for a corneal abrasion from a foreign body under the eyelid.
Started having headache on Monday July 27, began feeling nervous & dizzy. Went to urgent care to get checked out.
