EE was getting into a loader and as EE was pulling self up EE felt severe pain in right lower back.

EE was repairing the UFR pump when the pump cover closed without warning and pinched EE's left hand ring finger.

EE was working on the suction hose of the water pump. He was checking the bolts that were previously tightened. EE was tightening 6"" suction hose on water pump and felt a strain in his neck.

Struck by flying object

While looking up a rock flew into his left eye. Eyewash was used, then sought medical attention at Marshfield Clinic Wausau Center. Employee was treated for a left corneal abrasion. Given ""Tolner"" ointment for a 5 dat treatment.

Struck against stationary object

Swinging mall, missed and hit frame, his left hand hit the frame. Hand is sore, no swelling. Sought medical treatment on 7/6/05 with left wrist pain due to injury on 6/6/05.

Struck by flying object

While welding he got piece of metal or debris in his left eye. Small corneal abrasion to his left eye.
