employee was installing universal joint on 773 B Cat haul truck, Universal joint slipped out of spline off driveline. Striking employee on right side of head. Employee went to Grayson Urgent treatment center, Four staples were attached to close wound, employee returned to work with no restrictions.

Showing all 27Miner was manually removing bucket from skid steer and as the bucket released it fell striking miner in right shin. Miner was transported to medical facility and received 8 stitches.
Employee stated employee had experienced shoulder pain while working on screen deck.
EE was working on filling the newly installed greaser pump up on the Wa 600. When EE stepped back to see how full the tank was getting EE lost balance and fell hitting the floor. The rim of EE's glasses cut EE just above the eyebrow on the left side. Put a bandage on it from the first aid kit.
The date of the incident was 12/2/2019 and was reported by employee on 12/3/2019. EE said right shoulder was hurting, but wasn't sure they did it on the job. An accident report was filled out. After a visit to family Dr. and MRI, an orthopedic Dr. said the injury might be a new injury. EE reported this result to company on 01/21/2020 at 1:56 pm.
This was a truck driver driving a tandem dump truck. EE had a rock stuck in the tail gate and decided to open the tail gate with hands to get the stuck rock out. Then EE didn't move hand fast enough and the tail gate slammed closed on EE's finger.
It was a yearly Comprehensive Hearing test, which shows a Recordable Standard Threshold Shift of 25 db or more.
Performing close proximity maintenance on mill fan, hand contacted belt at pulley. Cut middle finger on right hand, needed 6 stitches.
Loading a shot, pulling the blue hose, felt pain in left elbow/forearm

Employee began working on the truck around 9:30 am. Employee performed a mental risk assessment. Removed the drive shaft from under the truck and put on tandems to work on. 1 of 2 caps were almost installed (with hammer) when driving the second cap in the drive shaft yoke employee struck their thumb with the hammer.
Employee was walking past a cone crusher when a 10"-12" rock bounced out of crusher striking the employee's hand against the handrail.
Employee was removing a return roller when his finger was mashed between return roller and conveyor frame.
Employee was filling bulk bags of mine safety dust (ground limestone powder). After filling bag employee removed filling spout and dust splashed into his face. Employee washed out eyes but was unable to remove contact from 1 eye and needed medical attention to remove.
Tech was installing a bucket on excavator using a dead blow hammer to drive in the flag pin. Paint chipped off the flag pin and struck ee's eye.
The employee was helping turn blow bars in Impact Crusher, he was standing inside Impact waiting on co-worker to lower bars to him with boom truck when he felt his right calf muscle pull in his leg. He said it didn't bother him at the time and called me around 6:30 pm at home and said it was getting sore. He went to doctor that night and got pain meds. He never missed any work.
Maintenance worker was cutting metal from hopper when hot slag landed on boot and burned through. Worker suffered burns to top of foot before he could remove boot.
It was a reportable hearing loss, the standard threshold was more than 25db on the 2000Hz on his right ear.
Just removed crusher base under which was a small rectangular opening through which power cable had run. Hole not noticed; blended in with expanded metal grating. Cleaning up work in preparation to barricade the crusher opening/area. Stepped into hole, slightly larger than a large boot footprint, leg dropped through, straining the knee of the other (left) leg and right shoulder.
Employee was stripping large diameter wire for installation in new motor control center when knife slipped and lacerated the palm of right hand.
Employee was performing fabrication (welding, grinding and chipping slag) and noticed irritation in eye. Employee was wearing PPE including welding helmet and safety glasses. Employee should have also utilized safety goggles.
EE was dismounting the machine and was observed using 3 point contact, lost his footing and fell approx 2 1/2 feet onto his side. The steps and hand holds were intact. There was a thin layer of fresh mud on the tracks and step surface from tramming the machine 10 minutes prior to the accident.
EE and another employee were removing a guard off the tail section of C-10 belt conveyor. Guard shifted and pinned his left hand against a metal beam.
EE was hooking up welding trailer, he squatted down to hook up safety chains and when he stood back up his left knee felt like it came out of the socket. EE went to ER and doctor told him his knee was fine and to RTW as scheduled. EE went to Dr approx 4 weeks later and set up for surgery. He had previously had surgery on knee from ATV accident when worked at Grassy Store.
Slip and fall on frozen ground, hit face on ground, got 5 stitches in forehead and 1 in his lip.
Employess of Barnette Contractors was a passenger in vehicle leaving jobsite; driver stopped to check on some equipment, leaving passenger in vehicle. When driver returned, contractor was out of vehicle. Driver found him lying in a ditch near vehicle. Autopsy has revealed a massive heart attack.
Employee was loading a Volvo hauler from a bin with 6" rip rap when he got struck by a piece of rock. The rock may have struck the left side of his hard hat causing a contusion to the left side of his head (temple area).
