It was a yearly Comprehensive Hearing Test, which shows a Recordable Standard Threshold Shift of 25 dB or more at 2000Hz in the left ear. *Retested and no threshold shift of 25 dB found.

Absorption of radiations, caustics, toxic and noxious substances

Tech was installing a bucket on excavator using a dead blow hammer to drive in the flag pin. Paint chipped off the flag pin and struck ee's eye.

Struck by flying object

Employee was loading a Volvo hauler from a bin with 6"" rip rap when he got struck by a piece of rock. The rock may have struck the left side of his hard hat causing a contusion to the left side of his head (temple area).

Over-exertion in lifting objects

The alleged accident resulted from employee manually lifting a partially filled 55 gallon onto the rear of a company service truck. In doing so, EE states that he pulled a muscle in the calf portion of his leg, which may have contributed to blood clot(s) in his leg. A portion of this blood clot(s) traveled to EE's lung causing him to be hospitalized.

EE WAS ASSISTING WITH A BLOCK @ THE PRIMARY CRUSHER WHEN THE ACCIDENT OCCURRED. THE INJURY HAPPENED AS EE PICKED UP A ROCK WEIGHING APPROX 100 POUNDS. AS HE LAID THE ROCK DOWN HIS FOOTING COUL D HAVE SLIPPED CAUSING THE ROCK TO LAND ON HIS LEFT HAND EE WAS UTILIZING PROTECTIVE EQUIP (GLOVES) TO PROTECT HIS HANDS.
