Subject was attempting to use a hoist device to manually open the doors on a mud bin. One of the hooks connected to bin failed, thus straightening out. The hook came free, striking the back left shoulder of the subject, causing a knot to form on the back of this shoulder. Subject went to the ER and was released and able to return to work.
Employee climbed a ladder in the sampling room to check on a plugged crusher. While working from the ladder employee allegedly twisted her knee and back.
The EE was installing rollers on a belt truss structure, when the strucure rolled over, resulting in a laceration to the back of his head.
EE was dressing out conveyor section on belt truss when the structure rolled over, due to unstable ground, causing sprains to EE's back and left knee.
Loading refuse, feed was shut off. EE got out of the trk to clean windows, rock fell off trk and hit him on the lip.
Air hose on air compressor came apart, hitting employee in the right hand causing contusion. This incident not reported until 7/18/07.
EE was setting a ladder in place and suffered a pulled muscle when the ladder slipped and he reached to catch it. EE did not seek medical treatment until 6/23/07.
Employee's foot slipped while he was climbing on head chute to cut crane loose. Employee twisted his back when he lost his footing causing muscle strain to lower back.
The EE was working the loadout sampler skirt boards. He had dropped one side and had loosened all the bolts on the other side, except for one. He had a socket and racket on the last bolt, when it broke. His right hand was propelled backwards and struck another bolt, puncturing his hand. He received 5 stitches.
Contr EE of Twin Ridge Development, cntr#b294. Acc investigated by MSHA. Refuse trk driver was struck by csx train while crossing track. Driver was fatally injured. MSHA has not determined whether it has jurisdiction/reportable since the accident location is owned by the railroad.Statement given at MSHA office 5/22/07.
EE was installing bent plate around screens in a kneeling position. Whe he stood up he experienced a catch in his right knee, causing pain & the inability to straighten it. EE missed 5/7 & 5/8 and 6/14 - 7/5/07 for knee surgery. This office notified of surgery and lost days 10/23/07. (addition to narrative per MM 10/26/07 #10)
Employee was connecting pipe and suffered lacerations to his head after falling from being hit in head with pipe. Employee lost consciousness for a short period of time but was well enough to return to work the next day.
Walking to ck belt on plant belt turned and headed back to picking table slipped fall Other employee went to talk to employee ask it needed ambulance. EE stated felt he could drive operation.
Employee slipped in the mud and fell, bruising his tailbone. He worked entire shift & sought medical treatment 1-24-07 and has not returned.
Employee laid torch on its side on corner of washer vessel. Torch fell off washer and hit employee on the head cutting him near his eyebrow.
Employee twisted his knee when he was unloading steel from truck and kicked a piece of material he thought was packing material but it was nailed to the ground.
