An unplanned roof fall was discovered in the intake of the 1st room set-up off of the 3rd South Mains. The location of the fall is at spad #9+80 between entries 8 through 10 in the cross-cut. The fall measured app. 60' long x 19' wide x 6'-8' high. This area is app. 800' from the #1 unit(MMU 001-0).

The section foreman was marking up sight lines in #2 entry of #1 MMU001-0, when a piece of rock fell between pins and struck him in the back. The rock measured 4' x 4' x 2". The foreman finished his shift and ensuing shifts, but began to miss work due to the injury on 10-05-2011.

EE was trying to remove a large rock from the tail of the Joy 14Cm15 by lifting the tail section to the roof and dragging rock off. A piece of the rock broke off and flew back 8'-10', striking the miner operator on the left lower leg and ankle.

An unplanned roof fall occurred in Panel 1-I at spad #21+70 in #2 entry and extending through the adjacent crosscut to #1 entry. The fall measured 70' long x 16'-18' wide x 8' high. No travel or ventilation was affected.

An unplanned roof fall occurred in the 1st West Main Parallels at spad #41+20. The fall was in the crosscuts between #7 entry and #9 entry and included the intersections of #8 and #9 entries. The fall was 150' long x 18.5' wide x 6'-8' high. No travel or ventilation was affected.

An unplanned roof fall was discovered at 7:30 am in 1-I panel at spad #21+70 in the cross cut between #5 and #6 entries. The fall started app. 10' from the rib line of #5 entry and extended into the intersection of #6 entry. The fall was app. 70' long, 19' wide and 10'-12' high.

Employee was changing a cell in a Ram car battery with a come-a-long. The chain slipped and he felt a pain in his left shoulder. He has since been diagnosed with degenerative changes in the shoulder and has history of prior treatment with chiropractic care on this shoulder. He worked for several weeks and has now had surgery on the shoulder.

Employee was driving through a back-up curtain on the supply road and felt a stinging pain in his left shoulder. He was later diagnosed with degenerative loss in his neck and shoulder.

EE was bolting. He had drilled the hole with the top steel and when he let the boom down to add the bottom steel, the top steel fell out of the hole and struck him on the cheek and ear, causing a laceration.

On May 4, 2011, EE allegedly injured ankle while walking on floor of a building. She completed her scheduled May 4 shift. The Ohio County Coal Co. is unaware of any medical provider who determined that she had to miss work as a result of alleged event on 5-4

A roof fall occurred in the 2nd North Sub-Main in #4 entry inby the intersection of spad #5+60 to outby spad #7+00 due to a hidden slip in the area. This is an area of old workings, no one was in the area and no ventilation was affected. The fall measured app. 120' long by 18'-20' wide by 8' high.

EE was installing a 6' glue bolt using a 2' wrench to push the bolt into the hole. The bolt bent and the wrench came off of the head of the bolt and struck him in the upper lip causing a small cut requiring 3 stitches.

At 4:25 p.m. an abandoned non producing oil well casing was hit on the # 1 unit in the #10 entry. A hole was made in the casing by a miner bit. A 300' barrier was left around the oil well location. The well was drilled in 1959 and location was spotted off of a state of KY. Oil and gas map.

Employee stated that while he was walking by the Joy CM-15, he tripped and fell into the miner. His right right left (upper thigh) struck a miner bit, which caused a laceration on his thigh.

EE was tramming a golf cart beside of #6 belt line while changing belt rollers. He drove through a low spot on the belt line and he and passenger tried to get lower in seat. EE's SCSR hung on timber and pulled his leg off of ride, leg struck timber and fractured femur.

EE and two other employees were setting a new belt header. Header frame was sitting on a duck bill of a scoop. Employees were attempting to level the header frame when EE crawled under the boom. The header suddenly tipped forward and struck EE.

A roof fall occurred in the #6 entry at spad 19+30 to spad 20+10 on the 002MMU in the 7th sub-panel of the 2G Panel. This fall was due to a hidden slip in the area. The height ranged from 4' to 6' high, the width was 18.5', and the length was 80'. The Stamler Ram Car, model PH10 co. no. RC302 sn. 1135, had rock on the back end of the batteries.

Employee was in the process of helping get a pin steel out of the pinner pot. Another operator was helping him. The operator did not know where he was because he was concentrating on the pin a nd chuck. For some unknown reason EE put his hand on the boom, the same time the other operator was raising the boom and caught his hand between the boom and short arm on the bolter.

Employee was attempting to dislodge a pinner steel that was hung in the roof. He for some unknown reason had his hand at the steel that was in the roof when he accidently hit his boom lever in the upward position causing him to mash his left hand between the roof and the bolter pot.

EE was going from #6 room to #5 room when he struck a roof bolt plate that was hanging down on a roof bolt that had been bent up and hanging down during the installation process. He contacted the plate with his left ear, causing a piece of the skin to be torn away and a cut on the inner front part of his ear.

EE was helping move the miner from #3 entry to #2 entry. He was on the corner of the #2 entry, they had put the pull strap on the head/bit of the miner to push the cable up into the face when the bit came out hitting him on the chin and collar bone.

At approximately 7:20pm in the #10 entry on #2 unit the miner cut into a hole that had water coming out of it. At that time it appeared to be a bore hole. At approximately 9:45pm the section foreman went into the #10 entry and he could smell crude oil. At that time it was determined that the hole must have been an oil well.

While the green ram car was positioning to dump, EE started across in front of the feeder. The corner of the ram car bed caught his right foot pushing him toward the feeder area. The ram car operator started to dump, not knowing EE was in front of his ram car, when he heard him screaming. The operator backed off of EE's right leg, ankle and foot.

A roof fall was observed by mine examiner in the return rooms of #1 unit. Roof fall locations is in 5th room set-up of 2nd North Main return at spad #16+10 inby to 18+20. Fall is 210 feet long X 18 feet wide X 45 inches high. This fall is in the old works. Fall is timbered around just incase someone needs to go close to the area.

EE was in the process of setting up the miner to cut coal. After setting up the miner, he went back to move the cable out of the haul road. He pulled on the miner cable to get it out of the road & felt a pain in his lower back. First day missed was 8/20/07. He will be getting an MRI to determine full extent of injury on 8/28/07.

Employee reported to his section foreman he twisted his knee at 7:30pm on 7/20/07, when he was hanging a cable in the #10 entry. He missed 3/27 & 28/08. He returned to work on 3/31/08. He started missing 4/1/08 and he had surgery on his knee 4/10/08. It was determined after his surgery it was work related.
