Employee stated that employee was tramming a scoop along the travelway and as employee was making a turn through a crosscut a 20' joint of 4" waterline caught the operator's compartment deck and sprung upward and hit upper portion of employee's back close to left shoulder.

Employee stated was setting up the bolter to install a resin bolt close to the left rib when a portion of draw rock measuring 24" x 21" and approximately 4" in thickness fell hitting EE's lower right side of EE's back.

A Roof Fall was discovered around 9:45 a.m. on No. 4 Belt, 33 crosscut in No. 4 entry. No employees were injured, no equipment involved. Ventilation controls were not affected. The fall measured 20' in width, 20' in length and approximately 10' in height. This fall will be cleaned up.

A ROOF FALL WAS DISCOVERED IN AN INTERSECTION OF #7 ENTRY AT SPAD #4147 IN THE PRIMARY ESCAPEWAY ON THE 4 BELT MAINS PANEL.THE FALL WAS APPROXIMATELY 7' IN HEIGHT, 19' IN WIDTH AND 20' IN LENGTH. ALL APPROACHES WERE DANGERED OFF,TIMBERS SET, CRIBS BUILT AND STEEL JACKS SET. NO EMPLOYEES WERE INJURED, NO EQUIPMENT INVOLVED. VENTILATION WAS NOT AFFECTED. THE LIFELINE WAS RE-ROUTED.

A roof fall was discovered in the intersection of No. 5 intake track entry at spad #3519. The fall was 20' in length, 20' in width and approximately 7' to 8' in height. No employees were injured. No equipment or ventilation controls were affected. This area was timbered off at all approaches. The lifeline was re-routed and management reviewed the changes with all employees.

A continuation fall was discovered by the fireboss. The falls encountered were connected by previous falls in the Old 3 West Mains at the following spad locations, 2059 (#1 entry), (#7 entry-2060) and (#5 entry 2043 to 2061). Timbers were set, barriers erected and danger signs hung to prevent employees from entering that area.

A roof fall has occurred at #6 break on #1 belt line (partial intersection and between block 20' in length x 19' in width and 5' in height). No employees were injured, no equipment was involved. Danger tags were hung and timbers were set at all approaches. No ventilation controls were affected by this incident. This fall will be cleaned up.

A roof fall was found near spad #2552. No employees were injured, no equipment involved, ventilation was not affected.Additional roof supports in the form of timbers were set to prevent reoccurrence. The fall was 50' in length, 10' in height, 19' in width. This fall will not be cleaned up. The lifeline and primary escapeway was re-routed.

Employee stated was walking by a rail supply car when a portion of rib rock measuring 24" in width, 18" in thickness and 20" in length fell hitting employee's lower left leg around the calf area.

An Ignition of methane has occurred in the No. 5 face on the 5 left panel. A methane bleeder had been cut into by the continuous mining machine. A spark created by a miner bit against the sandstone roof caused the ignition. The flash of methane was quickly extinguished by the water sprays which were on at the time of the incident.

A certified electrician was load testing a 6 volt battery on a 3-wheeler with a volt meter when a cap blew off from the center cell, exposing the electrician to liquid from the battery, toward their face and eyes.

Several roof falls have occurred on the old worked out panel from spad #'s 6719, 6717, 6696, 6701, 6716 2nd crosscuts outby the old faces. Also at spads 6680 in #3 entry and spad #6699 in #2 entry. Supplemental support in the form of timbers and cribs were installed. All approaches have been dangered off.

The mine roof collapsed on top and outby the MRS as it was being moved. The fall measured approximately 8 feet in height, 25 feet in length and 19 feet in width. A clean up and retrieval plan was submitted to recover the MRS. No employees were involved and no ventilation controls were affected by this incident.

A roof fall was discovered in the intersection of spad #6718 on the old 10 belt panel. The fall was approximately 7 to 8' in height and the entire intersection. Timbers were set and danger signs posted all approaches. No ventilation controls were affected. No employees or equipment were involved. A map of the area was sent to the local MSHA office.

Contractor employee was pulling a rubber tire supply car with a scoop. To avoid hitting a dewatering pump ee decided to set the scoop over against the right rib. As ee proceeded to tram the scoop, the back of the scoop deck undercut the rib causing some rib rock 30" x 14" and approximately 5" in thickness to dislodge striking ee on the back of ee's hard hat and shoulders.

2 roof falls discovered. 1st fall near spad 6577. 2nd fall near spads 6553, 6560 and toward spad 6573. No equipment or employees were involved. No ventilation controls affected. These areas have been timbered off and danger tags hung. Each fall was 19.5 feet in width and approximately 14 feet in height. A map of the falls were sent to the district office.

2 roof falls were discovered by the weekly examiner between spad locations 5918 and 5916, also from spad 6254 to 6256 on the old No. 10 belt panel. All approaches to the falls have been timbered off and danger signs hung. No employees or equipment were involved. No ventilation controls affected.

Employee stated EE was installing a 6' resin bolt when a section of draw rock fell from the mine roof striking the tip end of EE's left hand ring finger. The rock measured 18" x 18" and approximately 3 inches in thickness.

The D-10R dozer was being disassembled in preparation to be moved. The push-arms that had been removed were sitting on the ground. Employee stepped over one of the push-arms and it fell, striking right leg causing a fracture.

A roof fall was discovered near spad 600 and measured 35' in length 18.5' in width and approximately 7' in height. The fall was between #5 and 6 entry and did not fall into the adjacent intersection. No ventilation controls or escapeways were affected. No employees or equipment were involved. The area has been timbered off and all approaches flagged.

EE was helping 2 other employees lift a shuttle car speed reducer on to a 4-wheeler when EE felt something pop in right knee. EE works a 4 on 4 off schedule and was scheduled to return to work on 6/26/2017 in which EE's doctor stated was unable to return to work.

A roof fall was discovered on the Old 2 right panel off of 1 north near spad 1629. The fall measured 7' in height, 19'.5" in width and approximately 70' in length. The fall extended from 5 right to 6 right. No equipment or employees were involved. No ventilation controls were affected. Area was dangered and timbered off.

A roof fall was discovered by the fireboss in the right return of B-Panel of the old southwest mains. The length of the fall was approximately 110' by 18'5" in width and 8' in height. No ventilation controls were affected, no employees or equipment were involved. This area has been dangered off and timbers set to prevent access.

A roof fall was discovered by weekly examiner along the old No. 3 belt panel between spads 1034 & 1043. No equip or ee's were involved. No ventilation controls were affected. Area was mined in July 2010. 6' torque tension bolts were used. The fall measured 7' in height, 18.5' in width & 45' in length. Timbers were set at approaches & dandered off. Fall will not be cleaned up.

A roof fall occurred in No. 1 entry outby the face area. The fall measured approximately 18' wide, 8' in height and 60' in length. No ventilation controls were affected, no employees or equipment were involved. Cribs and timbers were set and the affected areas of approach were dangered off. There are no plans at this time to clean up the fall.

Employee stated was removing a rock off of the left side continuous miner measuring 12" in length 13" in width and 10" in thickness and as EE was going to roll it into the conveyor chain the rock shifted and rolled back onto left hand.

IE was operating Roof Bolter in 5 Right location in face. In Process of installing second row of bolts. When piece of draw rock fell from the roof between ATRS and drill canopy. The rock struck employee's hard hat and cut employee between hard hat and safety glasses. Rock measured 21 inches wide x 19 inches long x 6 inches thick.

An unplanned fall was reported on 12/18/2015 near spad No.4796. A continuation of this fall was discovered by the weekly examiner and a MSHA inspector traveling with him. The fall is 7' in height, 19' in width and approximately 120' in distance. No employees were injured, no equipment involved and ventilation was not affected. Cribs, timbers were set and the area dangered off.

An unplanned roof fall has occurred on the No. 1 Old North panel in No. 4 entry at spad location 1164 to 1172. This fall is approximately 60 feet long and 7 feet in height and 18 feet in width. 6 foot torque tension bolts were used when this area of the mine was developed in September 2010. No employees were injured or equipment was involved. This fall does not effect ventilation.

Employee was lapping up approximately 50 feet of line curtain when they felt a sharp pain in their lower back. The curtain was covered with mud and water at the time of the incident.

Employee was operating # 1 S.C. traveling up #3 entry, coming from the feeder, back to the left miner, when employee ran through a dip in the haulage way causing a pain between shoulder blades, in upper back. Employee visited local hospital due to strain to upper back.

EE was moving the miner cable out of the roadway when the bolt machine was backing up the bolter cable caught the employee's left leg and caused it to bend backwards injuring EE's left knee. This employee did not seek medical attention at the time of the incident. EE went to the doctor on 3/14/2016 in which it was determined EE could not return to work pending on an MRI.

Employee changed the water filter on the continuous miner and was putting the shield back in place with help from a coworker. As they dropped the shield down employee caught right finger between the shield and frame.
