Employee climbed into bed of semi with a shovel to break loose carbon that was stuck to the bottom of the bed, while using the shovel employee slipped and fell on right shoulder. Employee was taken to Express Care for evaluation, after x-rays it was determined no bones were broken. They recommend seeing an orthopedic doctor.

Mechanic was assisting the welder in picking up a plate of metal (4'x10'x1/2"). Two plates were stacked on top of each other on the ground. Employee tried to lift top plate so the forks of the bobcat could slide under top plate. When ee attempted to lift this plate, ee heard a popping sound from the lower right side of back. Employee collapsed onto the ground in pain.

while torching/cutting feeder chain during demolition at Crown II mine prep plant, chain struck worker on the hardhat causing slight laceration.

EE walked around timber trip that was stuck and struck a roof bolt with her hard hat. She had a neck stain at the time and continued to work until 10/9/08, when she had surgery.

EE was standing in last open X-cut in unit. A piece of rock fell from between the bolts, striking him on back of head and neck.

Employee was bolting at the face. He moved to the rear of the bolter when a piece of shale fell directly in front of him striking him on the upper right foot.

Employee was moving roof plates around on roof bolter when one slid down cutting his right lower arm. He received 3 stitches.

Notification by MSHA of possibility of pneumoconiosis.

Employee was struck on the back with a piece of slate.
