Near the end of his shift on 5/24/12, Employee stepped in uneven mine floor in an outby area and hurt his left ankle and foot, and his right knee. Employee did not fully report incident until 5/29/12. Employee did not begin missing work due to the injury until 06/06/2012.

Miner was preparing to move tracking system reader. When he cut the wire to the reader, the reader swung down and struck him in the face/mouth - striking his teeth. Employee did not miss work until June 8 thru 13, 2012.

AT APPROX. 2:45 AM ON THE MORNING OF 03/08/11 THE AIR BRAKE VALVE THAT OPENS AND CLOSES THE BRAKES ON THE HOIST FAILED. THE REPAIR TOOK APPROX. 50 MINUTES.

WHILE DOING PERMISSIBILITY ON # 34 MINER CABLE A PIECE OF ROCK FELL SMASHING HIS RIGHT HAND.

EMPLOYEE TRIPPED OVER A ROCK DUST HOSE WHILE DUSTING IN THE INTAKE. THIS ACCIDENT WAS DISPUTED FOR SEVERAL WEEKS BEFORE IT WAS DETERMINED TO BE A NON-WORK RELATED INJURY. THIS WAS NOT MSHA REPORTABLE.

While moving material on the roof bolter, a piece of rock fell striking him on his left hand. The impact of the rock broke his left pinky finger.

While hanging feeder power cable from roof, employee heard a "pop" and felt pain in his right shoulder. The employee subsequently had surgery on February 3, 2011, which was deemed "worked-related" by the attending physician. Because the employee was able to work for three months, this case was disputed. The employee returned to work on full duty status on April 18, 2011.

On the above date, employee was diagnosed with repetitive motion syndrome of the left upper extremity and believes it is work-related. No incident occurred that resulted in this condition. Th e illness was disputed until now.

A roof fall has occurred in unit #3 at crosscut #42 in entry #4. MSHA was notified at 0900 on December 31 2009. The area around the fall was re-supported and all approaches to the fall were propped. The travel way was also re-routed.

The employee had his hand on the tray of the roof bolter and a rock fell from the top, striking his hand.

While taking the rub rail off of a miner, the rail fell striking the employee in the shin.

Employee was putting two augers together, they weren't aligned properly and one of the augers sprung striking the employee's left thumb.

While hauling coal in a joy shuttle car, the operator struck the rib causing the rib to loosen and fall into the operators compartment, striking the operator.

EE WAS PINNED BETWEEN THE BOLTER BUMPER AND THE RIB, CRUSHING HIS RIGHT LEG BELOW THE KNEE.

Employee, complaining of back pain, was transported out of mine to receive medical treatment. He made no claim or assertion that he was injured at mine at time of incident. Upon investigation, management determined that incident was not work-related. MSHA issued 1046 citation. This 7000-1 submission is to abate citation #6675934.

Employee was walking alongside the roofbolting maching, stepped on a chunk of coal and twisted his right ankle.

WHILE MAKING THE INTAKE INSPECTION, THE FALL WAS FOUND AT X-CUT 78 ENTRY #8 MAIN WEST INTAKE.

Employee was installing boards on props to protect SCSR storage area when a rib rolled over and fell, striking him on his back knocking him to the mine floor. The coal rib also struck his feet. He worked the next two scheduled shifts but started missing on April 14. His first Dr. visit was April 15 when a left foot fracture was found after xrays were taken.

Installing a curtain board on the mine roof with roof bolting machine, when on side operator pressured ATRS against roof, the floor jack pressed employees right foot into the mine floor. His right foot suffered a contusion from the middle to the distal end.

Employee was unpluging a battery from a ram car when he slipped and fell while standing on loose coal. His left thumb was cut when he fell. Three stitches were required to close the wound.

While washing a Roof Bolting Machine he stepped in a hole and twisted his left knee. He did not begin missing until April 2, 2009 after a surgical procedure was preformed on his left knee.

Unintentional roof fall, #78 crosscut #4 entry unit #3 North Submain. Length 25' width 18', height 8'. Fall propped all 4 sides, 4 way intersection. Date of development 1-5-09.

Employee was going through a 3x3 mandoor in a stopping when dust flew into his left eye. He finished the shift but was kept off work due to an abrasion on his left eye.

Employee was assigned to work on the conveyor belt. As he was traveling (walking) in a x-cut that contained muddy gob on the floor he slipped on a 3" water pipe and fell onto his buttock. He was taken by ambulance to a hospital and treated for a contusion.

Man was roof bolting, piece of top fell while he was drilling. Put his left hand up to protect himself and was struck on left hand and side of his face. Received abrasion to face and laceration to left hand. 2 stitches on hand.

Unintentional roof fall 11th Main West Entry at 0 x-cut on the bottom. Will be proped off from area people can walk. Length 25', width 15', height 8'. 4 way intersection. Approx date of mining June 2004.

Installing an 8 ft roof bolt, while straighting the bolt to install he slipped and strained his back.

Employee set lunch on skid on ground, raised up and bumped into roof bolt. Loose debris and one 6" flat coal piece rolled down back.

A roof fall occurred in the 2nd South in entry 3 at x-cut 18. The fall measured 20' long, 25' wide and 8 feet high. It did not cause injury, affect travel, ventilation or equipment. The fall will not be cleaned up.

Employee was backing a personnel carrier into a crosscut when the tongue of a wet duster struck and lacerated his left leg just below the outside of his knee. 12 stitches were used to close the wound.

Employee was operating the continuous miner loading a ram when a rib fell striking the employee on the back of his right leg and knocking him into the ram car. He continued to work until Nov 18, 08 until which time his physician began treating him for an infection in the affected area of his right lower leg.

A fall occurred in the 2nd west submain in entry #4 at X-cut #33. The fall measured 25' long, 30' wide and 7' high. The fall took out a stopping and caused some leakage. It did not affect travel, cause injury or damage equipment. The fall will not be cleaned up.

A roof fall occurred in the North sub main in entry #9 x cut12. The fall was 40ft long, 20ft wide and 8ft high. This fall did not cause injury, affect travel, ventailation or passage of people. The fall will be cleaned up.

While roof bolting the drill steel plugged up; once cleared we began to raise the drill steel to the roof. When the steel struck the roof a clod fell and the steel bent and snapped back striking his left hand.

A roof fall occurred in the 2nd south at X cut 6 in the entry #3. The fall measured 30ft long, 20ft wide and 81/2ft high. The fall did not cause injury, passage of people or ventilation. The fall will not be cleaned up.

Employee reported that he tripped and lost his balance when he fell and twisted his right knee.

While in the process of roof bolting, a piece of clod fell from the roof. As the miner was moving out of the way of the falling clod it struck him on the left elbow (posterior). His elbow was swollen and cut in two small places.

A roof fall occurred in the South Sub Main Entries xcut 33 in entry #12. The fall measured 25' wide, 25' long & 5' tall. It did not cause injury, affect ventilation or passage of people. No equipment was affected. It will not be cleaned up.

Occured in Main West, intake x cut 119 entry #8 fall measured 20'l x 20'w x 8'h. did not affect ventilation, travel on cause injury and will not be cleaned up.

Twisted left wrist. 2" water pipe fell of sled, while trying to reload, he twisted his left wrist. Did not begin missing until Jan 28, 2009 due to surgical procedure to repair the damaged wrist.

A roof fall occured in the South Sub Main, entry #3 at x-cut 40. The fall measured 20' wide x 20' long x 8' high. It did not affect ventilation travel or cause injury. The fall will not be cleaned up.

Employee was hit in the face with a piece of steel.

IE was working demolition of a steel structure at the bottom of the slope. He and another employee were torch cutting 8" channel iron into 36" long pieces. IE cut one piece off and set it down on the ground, leaning it up against the belt frame. Soon after, the piece tipped and fell against IE's lower rt leg, causing an approx 3" long laceration to his lower rt leg.
