Roof fall at No. 4 entry at spad 257 on 1 south mains. Fall was 18'L x 20'W x 2'5' thick, did not clean up fall-area dangered off. Fall was in a non mining area and no airway blockage. Area was bolted with 5' torque tension bolts.
Roof fall at No 2 entry between 20 & 21 x-cut on 1st left panel off of 1 west mains. Fall was 60' L x 20' W x 5' thick. Did not clean up. Cribbed & dangered off. Fall was outby section. No airway blockage. Area was bolted with 5' torque tension bolts.
Individual was installing roof bolts when he was struck on the left wrist by a piece of drill steel. He suffered a nondisplaced hairline fracture to his left wrist. He did not cease work to his left wrist. He did not cease work and was treated later and placed in a wrist splint.
Roof fall at No 5 entry between 15 & 16 x-cut on 2 south mains. Fall was 20' wide X 50' long X 5'-9' thick. Cleaned up fall and rebolted area. 10 day reporting window extended due to mine holiday and vacation.

Area bolted with 5' torque tension 3/4" roof bolts. Immediate roof was shale with laminated sandstone above it. Fall inby loading point.
Roof fall at No. 11 x-cut, No. 9 entry of 1 west mains. Fall was 20' W x 35' L x 3' - 5' thick. Cribbed and dangered off. Will not clean up. Area bolted with 5' torque tension 3/4" roof bolts. Immediate roof was shale with laminated sandstone above it.
EE was hanging a water line when he slipped and struck his head on the rib. EE suffered a laceration to the right side of his head, which required 4 sutures.
EE states he was in a squatting position grinding metal, when he felt back pain and hip pain when standing. EE sought medical treatment on 7/2/09 and was taken off work by his treating physician.
The employee was unhooking a piece of pipe connected to the bucket of an excavator when the operator engaged the emergency stop lever so he could move the bucket. The operator didn't realize his leg was depressing the bucket joystick. The bucket dropped onto the employee's left hand, pinching his fingers. Surgery was performed & ee is off work.
Employee states that he was tramming up an incline with a beam on the forks of a forklift when the forklift stopped running causing brakes, steering & hydraulics to fail. The forklift rolled backwards, caught traction with tires & proceeded to roll over on it's top. Employee suffered head contusion.
Employee states he felt low back pain while he was unloading a truck bed.
EE was assisting in the installation of a jib on a 90ton crane, while standing on a ladder, the jib fell off the crane and onto the ladder, knocking the ladder over and the employee fell to the ground, suffering a contusion to his lower back and tailbone.
Employee states he was getting steel beams ready for crane lift and felt low back pain/left leg pain when he bent over and lifted end of beam. He delayed seeking medical attention to see if pain would ease alone. Condition did not improve and treatment was sought 10/24/08.
