Employee was helping change a screen. They were using the overhead crane to move the screen. Employee was watching the screen to make sure he didn't get under the suspended load when he lost track of his surroundings and fell down 12 steps to the concrete floor.
Employee was fire-bossing track and power centers. As he was walking to box, he stepped on a small unseen ledge and slipped when he fell to the ground and began to experience pain in his knee.
Crew was spot bolting 5R on #2 section. Both operators had finished putting up bolts, and operator went to inch tram controls. Offside bolt man walked back to get bolts for his side. The operator started the bolter and began tramming forward, that is when the bolter slid off a small ledge, on top of employee's foot.
