Employee was working on a Joy 14-9 continuous miner with the top lid opened when the lid fell down on middle finger on the right hand. This action fractured the tip of the middle finger on right hand. Employee took self to the doctor after the shift and returned to work the next day with no time lost.

Crawling, not lifting properly. *Employee did not notify mine management of any injury.

Was going to move a water hose out of road. Tripped and fell on frozen ice and debris

EE was installing a section of 6 inch water line. He reached across belt structure to pick the water line up and felt pain in his groin and abdomen area.

Workman was standing on the #7 head drive pulling on motor leads when he lost his footing and fell into a H beam on the head drive. He strained his right shoulder and lower back.

Employee was shoveling the #2 overland belt when he struck his hand against a metal trough. The metal sliced through the glove he was wearing and lacerated his right little finer. Two stitches were required to close the wound.

EE turned the small coal sieve to retrieve any magnetite from the water where coal had been washed. This sieve rests on a pivot stand and is turned weekly. EE was placing the sieve into its working position when he strained his back. He continued to work until 6/2/14 when he sought medical attention. EE was taken off work effective 6/3/14.

The employee was making up roof bolts and plates to bolt a cut when he slipped and his arm struck a roof bolt causing a laceration that required 7 stitches to close the wound.

Employee was applying rockdust in the #8 Head Drive Area when the waterline ruptured and a piece of the plastic struck him in the left side of the head causing a laceration that required stitches. On 6/11/13 we received information in the form of a work excuse taking ee off work beginning 6/10/13.

EE was repairing the discharge line on a Floc transfer pump. Floc was present on the floor and he slipped and struck his head on an elevated piece of concrete. EE sustained a laceration requiring 5 staples to close the wound.

EE was hanging cable around the tailpiece when the waterline blew apart at the head drive. The waterline pushed forward and the water manifold at the tailpiece struck EE in the back and fractured a rib.

He was dragging ventilation curtain from #6 entry to the #5 entry and as he turned the corner of the #5 entry he came in contact with a roof bolt plate.

During retreat mining we had a pillar fall occur that extended from the area where pillars had been mined in the normal cycle of retreat mining and extended outby where the continuous miner was cutting coal out of a pillar while performing retreat mining.

Employee was trouble shooting a hydraulic problem in the valve chest on the continuous miner. A valve stuck and the bumper on the miner sat down on his left foot. Employee sustained 3 fractured toes and a laceration requiring stitches.

EE was helping lift the arm on a head drive when the arm swung around and his finger was caught in a pinch point. EE sustained a laceration requiring stitches.

Employee was using a slate bar to move the track when the bar slipped and came back hitting him in the side of the face causing a fracture of the zeugmatic arch.

EE was cleaning crusher chute when coal broke loose covering his feet, causing him to fall back hitting his head and right elbow on grating which resulted in a laceration on his head requiring 10 stitches.

Loading Hi-line in scoop, he grabed pump cable and felt shock in arm, hand and down right side. He did not seek any medical treatment. He went back to work the next day.

EE was setting timbers. A co-worker tossed him a timber and it bounced and landed on his right foot.

Employee struck his left ear against torque tension bolt lacerating his ear which required.

Employee was trimming belt skirting, when the knife went through material and cut his right index finger, requiring 18 stitches.

A roof fall occurred in the intersection 1 break out-by spad #944 in the # 3 entry.The fall measured approx. 19'W X 25' L. X6' Thick. Persons, ventilation nor equipment were effected. This area will not be rehabilitated.

A roof fall has occurred in the #6 entry on the northwest panel #5, 150 feet inby spad # 1788. It fell betrween the blocks between the last and nect to last break forming the bleeder across the panel. The fall measured 18 ft wide, 50 feet long adn 3 to 5 feet high. Persons, ventilation, equipment nor travel were impedded. This area wil not be rehabilitated.

A roof fall occurred at the bleeder, next to the last open crosscut between #2 and #3 entry. The fall measured 18 feet wide, 50 feet long & 6 feet thick. Persons, equipment nor ventilation were effected. This area will not be rehabilitated.

Employees back started hurting after hanging car anchor. Placed on restricted duty.

Employee was hammering bull gears on head drive when his knuckle hit gear teeth lacerating finger on right hand requiring a staple.

Changing battery on permissable ride when battery lead grounded against the frame causing the battery to explode resulting in a chemical burn to one eye.

A non-injury roof fall occurred on the active 001 & 002 MMU in the last open crosscut in the #4 intersection. The fall measured 19' wide x 10' thick and 260' long. No persons, equipment, nor ventilation were affected. This area will be rehabilitated.

Employee stepped from ladder on top of tank when the tank collapsed. He grabbed the edge the tank, cutting his fingers and resulting in three stitches.

EE was helping with belt move and was assisting with lifting part of the belt structure when he felt a pull in his lower back. Physician took him off work effective 10/05/2009.

EE was lifting a man trip with a jack when the bar came out and struck him in the left jaw.

Employee was outside loading top structure into scoop when he tripped on a cable and his back.

Employee picked up nail board and turned around to empty it when he felt pain in his lower back and placed on restricted duty.

EE was cleaning out chute, when his hand slipped on round bar and cut his finger, requiring stitches.

EE was tramming roofbolter with his right hand on top of the canopy, when his fingers were mashed between the canopy and the mine top, resulting in an avulsed finger tip on his right middle finger and required stitches on his right index finger.

A roof fall has occurred on the 001-0 MMU, 1 break inby spad #601. The fall measured 19' wide x 40' long x 5' thick. Persons, ventilation nor equipment were effected. This fall area will not be rehabilitated.

Employee was riding rail runner in low clearance and hit his back on the top causing injury to his back and ribs.

Employee was in a shuttle car heading to miner when it hit a hole and threw his head into the canopy.

Employee was using a slate bar to close the door on a railroad car when the slate bar broke about ten inches from the end. It struck him on the right lower leg causing a fracture to the tibia and fibula.

Employee was operating a three wheeler under a low roof when he hit a rock and tried to his head down. When he ducked his head, it hit the battery lid causing a laceration which required two staples.

Employee was hitting broken shaft with hammer to get it out of feeder when a piece of metal chipped off and went into thigh. Employee voluntarily quit before returning to position.

Employee was aligning the bolt in the drill pod and accidently activated the rotation lever. His glove caught on the roof bolt and twisted his hand. He was taken to Pikeville Medical Center and x-rays revealed a fracture to his left middle finger.

A roof fall occurred 700' outby the working section in the return airway on 002-0 MMU. Person, ventilation, nor equipment were effected. This area was cribbed up and dangered off.

A Coal bank that was not mapped was mined into. No injury or illness resulted and ventilation nor equipment was affected.

Was trying to dislodge rock in wash box reject chute with a slate bar when his hand hit the edge of chute opening causing a laceration on his finger requiring stitches.
